Position
Brief Summary: Under the direction of the Manager of the Adult Day Program & Geriatric Outreach Services, the GOT Clinician plays a pivotal role at Unionville Home Society. You will have the opportunity to deliver clinical services to support the Geriatric Outreach Team.
Essential Job Functions:
- Maintains an awareness of community resources, and identifies gaps in community services.
- Conduct comprehensive Geriatric Assessments (CGA) of an older adult including evaluation of medical conditions, cognitive status, functional abilities, mental heath and social supports.
- Identify and manage common geriatric syndromes (e.g. falls, frailty, polypharmacy, incontinence, multimorbidity)
- Assess risks related to falls, cognitive impairment, self-neglect, and caregiver burnout
- Works within an interprofessional team (e.g. nurse, social work, occupational therapists, physiotherapist, physicians)
- Refers clients to other specialized services/linking with appropriate resources
- Works in a manner to ensure safety to oneself, clients, colleagues and the work environment
- Advocates for the client and families
- Provides clinical support to Intake and Teletriage for referrals as required
- Participates in Outreach Team conferences and business meetings
- Maintains positive working relationships with community agencies/partners
- Engages with community partners and provides updated GOT service information
- Assists in developing new manuals and material as required
- Communicates appropriate information to service providers, clients and their families
- Advocates for the client and families as needed
- Provides timely feedback to referral source
Qualifications
- Valid driver’s license and availability of a car
- Member in good standing with respective College
- Community experience on in-home assessment, an asset
- Empathetic attitude and desire to contribute towards the well-being of seniors
- Organizational skills to manage timetables and responsibilities
- Ability to exercise good judgment, tact and diplomacy
- Empathetic attitude and desire to contribute towards the well-being of seniors
- Written and verbal communication skills related to the position
- Good interpersonal skills
- Experience in long term care setting is an asset

Pay: $34.00-$43.00 per hour
