Founded by Urologist Dr. Alaina Garbens (MD, PhD, FRCSC), Élevé is a physician-led, evidence-based women’s health and regenerative medicine practice bridging the gap between pelvic care, sexual wellness, hormonal health, and regenerative aesthetics. We operate under a “no silos, no shame” philosophy, offering patients a sophisticated, evidence-based, and multidisciplinary approach to their most intimate health concerns.
Élevé Midtown is a new women’s health division within the SuperForm integrated health campus in Midtown Toronto. Élevé Midtown is a new women’s health division within an integrated health campus in Midtown Toronto, alongside FORM Face + Body, Form Hair, a Regenerative Health & Longevity Division, and a Men’s Health Division. The Patient Care Coordinator is the connective tissue of this division, the person who ensures every patient moves seamlessly from assessment to program enrollment to treatment, and that nothing falls through the cracks.
The Opportunity
This role sits at the intersection of patient care and clinical operations. You are the first person a patient speaks to after their assessment, the person who walks them into a treatment program, and the person who keeps the division running day to day. You will manage patient flow, lead enrollment conversations, coordinate scheduling across the clinical team, and handle the administrative backbone of a growing medical division.
This is not a front desk role. You will be trained on Élevé’s clinical programs and pathways so that you can have informed, confident conversations with patients about their care options. Your ability to build trust, communicate clearly, and guide assessed patients into the appropriate program is central to the division’s success.
Prior experience in a medical or aesthetic clinic is a strong asset, but we will train the right person on our programs, systems, and approach. What matters most is your ability to connect with patients, stay organized under pressure, and take ownership of the patient experience from end to end.
What You Will Do
Patient Enrollment & Program Coordination
- Receive patient handoffs directly from the NP immediately following each assessment, ensuring no delay in transition of care
- Lead private, unhurried enrollment conversations, guiding patients through their assigned pathway and program options with clarity
- Maintain full knowledge of all programs, including structure, session count, pricing, and inclusions, to provide accurate and confident responses
- Present program options clearly and consistently using approved materials and messaging
- Address questions or concerns with empathy and structure; when cost is a barrier, offer established payment plan options rather than discounts
- For patients not ready to proceed, schedule a defined follow-up date and time to maintain continuity
- Track and document all assessment outcomes daily, including enrollments, pending follow-ups, and reasons for non-enrollment.
Scheduling & Patient Flow
- Manage the division’s booking system — assessments, NP treatment sessions, physiotherapy, and Dr. Garbens’ procedure days
- Pre-load Dr. Garbens’ schedule before each on-site visit: cases prepared, patient files ready, procedure room requirements confirmed
- Coordinate patient flow throughout each day so that wait times are minimal and handoffs are smooth
- Manage appointment reminders, follow-up sequences, and re-engagement for patients who have not yet enrolled.
- Process cross-referrals from other campus verticals (aesthetics, hormones, urology, men’s health) and ensure referred patients are booked promptly
Administrative & Clinical Support
- Manage patient intake: registration, consent forms, intake documentation: ensuring everything is complete before the assessment begins
- Maintain patient records in the EMR, ensuringaccurate, current, and compliant with privacy requirements
- Process payments, manage payment plans, and maintain accounts receivable — no outstanding balances beyond 30 days
- Track and reorder consumables and retail inventory against defined par levels — flag reorders before stock runs out
- Manage retail product sales (pelvic health products, wellness items) and maintain the retail display
- Prepare and organize treatment rooms for daily operations and for Dr. Garbens’ procedure days
- Coordinate with shared campus staff (reception, MOA) to ensure seamless patient arrivals and front-desk experience
- Maintain supply storage, clinical organization, and daily opening/closing checklists
Reporting & Communication
- Participate in daily team huddles (10 minutes, start of each day) reviewing daily schedule, any flags from yesterday, program enrollment pipeline status
- Maintain program enrollment tracking logs and share weekly summaries with the operations lead
- Respond to all patient inquiries by phone and email within 24 hours. promptly, professionally, and on-brand
- Communicate patient concerns to the NP or Dr. Garbens as appropriate: know when to escalate and when to resolve directly
- Support monthly practice reporting: patient volume, program enrollment rates, revenue by pathway, and referral source tracking.
What We Are Looking For
- Experience in a medical, aesthetic, or wellness clinic environment, you understand how a clinical practice operates day to day
- Strong communication skills with a natural ability to build trust and rapport quickly, particularly around sensitive health topics
- Comfort with enrollment and program enrollment conversations: you can present program options confidently without being pushy
- Excellent organizational skills and attention to detail: scheduling, documentation, inventory, and follow-up must be airtight
- Familiarity with EMR systems and clinical administrative workflows (training provided on our specific systems)
- A professional, polished presence that reflects the Élevé brand: clinical precision, warmth, and quiet confidence
- Discretion and maturity in handling confidential patient information and sensitive clinical content
- A team-first mindset: willingness to support the NP, physiotherapist, and shared campus staff as needed

Pay: $35.00-$45.00 per hour
