Summary
The Mobile Wellness Case Manager utilizes advanced clinical knowledge to contribute to the efficiency of service delivery and advancement of practice and interventions on the Mobile Wellness Team. The Case Manager provides services at various sites in the community including private homes. Services offered include assessment, intervention, treatment, outreach and/or consultation, and referral for clients who have a Serious mental illness and their caregivers/families.
The Case Manager understands the available community services and resources and demonstrates strong system navigation skills. Case consultation, case conferencing, and advocacy with community partners and service providers are integral components of this role.
Working within an interdisciplinary team and guided by the Seven Grandfather Teachings, the Case Manager collaborates and consults with internal and external partners to facilitate referrals, coordinate care, and ensure service continuity. The Mobile Wellness Program provides specialized services to individuals experiencing serious mental illness; therefore, the Case Manager must possess an in-depth understanding of treatment pathways, crisis intervention, and recovery-oriented approaches.

Case Management Responsibilities
- Provides advanced clinical practice and interventions in the community setting
- Screens and responds to referrals through the intake process, and from a variety of sources, to ensure clients meet service mandate; provides mental health and/or substance abuse assessments and, where appropriate, takes steps to stabilize crisis situations and makes referrals to follow-up service and support
- Coordinates client treatment by ensuring that clients have a service plan that maximizes rehabilitation and utilizes relevant community supports that are developed in partnership and coordinated with the client and caregiver/family and the interdisciplinary team, including assisting with life skills, symptom management, and readiness for change in living, working, and social environments
- Promotes and facilitates client independence, by modeling, counseling and teaching life skills within a bio-psycho-social rehabilitation model
- Documents observations, interventions and service outcomes in client records; reviews and maintains up to date clinical records to monitor and evaluate treatment outcomes
- Collaborates and consults with other service providers on the interdisciplinary team by providing input and feedback
- Participates in and delivers educational sessions for clients and their families/caregivers and community groups as assigned
- Develops community support components to service plans that maximize the client's inclusion in the community in collaboration with client, family, and other sources of support for the client, acts as client advocate
- Communicates and shares information with community agencies and the public about mental illness and mental health and/or substance use services, promoting good public relations and facilitating efficient and cooperative relationships
- Performs other related duties as required
General Service Responsibilities
- Complete all charting, documentation, and data entry within the Electronic Medical Record (EMR) system accurately and in a timely manner
- Compile and submit reports, statistics, and quality indicators required by funders and the organization
- Ensure strict adherence to privacy, confidentiality, and PHIPA/FIPPA standards
- Adhere to organizational policies, procedures, and professional standards of practice
- Maintain active membership and compliance with the standards of the applicable regulatory college
- Demonstrate adherence to traditional protocols and cultural sensitivity when providing services within First Nation communities
- Complete all required leave documentation (e.g., annual leave, professional development, certified or uncertified sick leave, overtime, etc.) in a timely manner
- Participate in professional development, certification, and training as required by Noojmowin Teg Health Centre, funders, or legislation
- Attend staff meetings, strategic planning sessions, wellness activities, annual general meetings, and other required organizational events
- Perform other duties as assigned
Qualifications
Mandatory
- Completion of a post-secondary education in social services or related field such as social services work
- Registration in good standing with the Ontario College of Social Workers and Social Service Workers or an equivalent professional regulatory college
- Minimum of three (3) years of experience providing clinical or community-based services within a mental health and/or addictions setting
- Demonstrated experience in case management, crisis response, and service navigation
- Valid Class G driver’s licence, access to a reliable vehicle, and a clear driver’s abstract
- Clear Vulnerable Sector Criminal Record Check
Preferred
- Experience working with First Nation communities and/or Indigenous organizations
- Familiarity with electronic medical record systems and data collection processes
- Knowledge of community resources and service providers within the Manitoulin Island region
- Ability to speak or understand Anishinaabemowin considered an asset
Knowledge, Skills, and Abilities
- Strong clinical judgment and assessment skills with the ability to triage and manage complex cases
- Demonstrated knowledge of serious mental illness, concurrent disorders, and trauma-informed care
- Excellent interpersonal, communication, and advocacy skills
- Ability to work collaboratively as part of a multidisciplinary team and maintain positive relationships with community partners
- High attention to detail, organizational ability, and strong documentation skills
- Ability to work independently with minimal supervision, managing competing priorities in a fast-paced environment
- Knowledge of applicable legislation, such as the Mental Health Act
Work Conditions
- Services are delivered across the Manitoulin Island district
- Frequent travel and occasional evening or weekend work may be required
- Combination of office-based, community-based, and mobile/outreach service environments
- Exposure to emotionally distressing situations may occur; self-care practices and debriefing supports are available
