Women’s Health in Women’s Hands Community Health Centre (WHIWH CHC) provides racialized women, trans and non-binary clients from the African, Black, Caribbean, Latin American, and South Asian communities in Toronto and surrounding municipalities with culturally safe, relevant, and responsive primary healthcare. About TELUS Health MyCare – Union Clinic
TELUS Health MyCare , Union Clinic is a primary care clinic located in downtown Toronto and is home to seven family physicians providing comprehensive primary care services. The clinic supports patients through a combination of in-person and virtual care and offers a broad range of services, with clinical areas of focus including women’s health, mental health, sports medicine, ENT, dermatology, and chronic disease management. The clinic is continuing to expand its service offerings, including the planned introduction of travel medicine services. Patients also have access to additional healthcare professionals through the TELUS Health MyCare platform, including dietitians, menopause specialists, weight management specialists, and counsellors
About the IPCT Partnership
Women’s Health in Women’s Hands Community Health Centre (WHIWH CHC) is the lead organization for the Interprofessional Primary Care Team (IPCT) Initiative, a provincial investment designed to improve access to comprehensive, team-based primary care services.
Through this initiative, WHIWH CHC is partnering with TELUS Health MyCare – Union Clinic to expand access to primary care services and strengthen interprofessional care delivery. The successful candidate will be employed by WHIWH CHC and embedded within the TELUS Health MyCare clinical team through this innovative partnership.
The role is governed by WHIWH CHC’s terms and conditions of employment, while day-to-day clinical practice will occur within the TELUS Health MyCare environment. The Registered Nurse will work collaboratively with physicians, administrative staff, and other healthcare professionals to support patient access, preventive care, chronic disease management, and high-quality patient-centred care. Care may be delivered through a combination of in-person and virtual modalities, as appropriate
Position Summary
The Registered Nurse will play a key role in expanding access to comprehensive primary care services through the Interprofessional Primary Care Team (IPCT) Initiative. Working within the TELUS Health MyCare interdisciplinary team, the Registered Nurse will support preventive care, patient intake, chronic disease management, immunization services, health promotion, and urgent care workflows.
The role will contribute to improving patient access, continuity of care, and clinic flow by conducting clinical assessments, supporting preventive screening initiatives, coordinating follow-up care, and assisting with a range of nursing and physician-supported procedures. The successful candidate will work collaboratively with physicians and the broader care team to support efficient clinical operations and improve patient outcomes.
RESPONSIBILITIES:
Clinical Care and Patient Assessment
- Conduct nursing assessments, health histories, triage, vital signs, clinical observations, and patient intake activities.
- Perform comprehensive and focused assessments for adults, infants, children, prenatal patients, and individuals with chronic health conditions.
- Support patient flow through urgent, same-day, and walk-in care services by obtaining initial assessments and clinical information.
- Perform physical assessments, health education, counselling, history-taking, and other nursing activities in accordance with College of Nurses of Ontario standards of practice.
- Provide advocacy, patient education, lifestyle counselling, and support for patients and families.
Preventive Care and Immunization Services
- Prepare and administer vaccinations in accordance with current public health recommendations and organizational guidelines.
- Support vaccine counselling and patient education regarding vaccine benefits, risks, and common misconceptions.
- Monitor vaccine inventory and support cold-chain management processes as required.
- Participate in preventive screening, recall initiatives, and patient follow-up activities to support population health management and early detection of disease.
- Promote health, wellness, disease prevention, and evidence-informed self-management strategies.
Women’s Health, Child Health and Clinical Procedures
- Support cervical screening and other preventive health assessments.
- Assist physicians with procedures including IUD insertions, ear syringing, cryotherapy, and other office-based procedures.
- Conduct baby measurements and support well-baby and well-child assessments, including height, weight, growth monitoring, and developmental screening.
- Support pregnancy intake appointments through collection of relevant clinical information and preliminary assessments.
- Perform nursing procedures and interventions within scope of practice, including specimen collection, wound care, medication administration, and point-of-care testing as appropriate.
Chronic Disease Management and Lifestyle Counselling
- Support patients with chronic disease monitoring, including hypertension, diabetes, asthma, dyslipidemia, and other chronic health conditions.
- Provide lifestyle counselling related to nutrition, physical activity, smoking cessation, weight management, and disease prevention.
- Participate in chronic disease education, medication review, follow-up, and care coordination activities.
- Collaborate with physicians and other healthcare providers to support evidence-informed chronic disease management and continuity of care.
- Facilitate referrals to internal and external healthcare services and community resources, as appropriate.
Documentation, Quality Improvement and Collaboration
- Maintain complete, accurate, and timely documentation within the electronic medical record system.
- Participate in chart reviews, quality improvement initiatives, care planning, and case discussions.
- Collaborate effectively with physicians, nurses, administrative staff, and community partners to support integrated patient care.
- Participate in the development, implementation, monitoring, and evaluation of clinical and health promotion programs and services.
- Contribute to a positive, collaborative, and patient-centred practice environment.
- Perform other duties as assigned.
QUALIFICATIONS:
- Current registration and license in good standing with the College of Nurses of Ontario. Baccalaureate degree in Nursing from a recognized university.
- Minimum three to five years of nursing experience, preferably in primary care, family practice, community health, ambulatory care, or a related healthcare setting. Demonstrated proficiency in comprehensive health assessments, problem-focused assessments, triage, patient education, and chronic disease management.
- Experience administering vaccinations and supporting preventive care initiatives.
- Experience supporting women’s health, child health, and office-based clinical procedures is an asset.
- Demonstrated knowledge of health promotion, disease prevention, and patient-centred care. Demonstrated ability to work effectively in an interdisciplinary healthcare environment.
- Experience supporting quality improvement and patient flow initiatives is an asset.
- Strong communication, organizational, and problem-solving skills.
- Experience with electronic medical records and clinical documentation systems.
- Demonstrated commitment to equity, diversity, inclusion, anti-racism, and culturally responsive healthcare.
- Experience working with equity-deserving populations is an asset.
REPORTING:
The Registered Nurse will be employed by Women’s Health in Women’s Hands Community Health Centre and deployed to TELUS Health MyCare – Union Clinic through the Interprofessional Primary Care Team (IPCT) Initiative. Administrative supervision will be provided through WHIWH CHC, while day-to-day operational supervision, clinical integration, and collaboration will occur within the TELUS Health MyCare practice environment.
