Position: Clinical Case Manager - Services to Indigenous Population

Location: Sarnia Office

1 Permanent Full Time

Preference given to Indigenous Candidates

Reporting to the Manager of Integrated Services, the incumbent will be responsible for the following functions:

Providing client needs identification through ongoing assessment and client directed care planning.
Implementing client centered plan of care that focus on strengths, wellness and empowerment to support recovery and goal achievement.
Providing crisis prevention and intervention planning with the hope of preventing a relapse and/or hospitalization.
Acting as a resource person or an advocate to assist individuals in obtaining information/access to services/supports which are deemed needed.
Liaising with family/significant other and natural supports to facilitate the clients’ links within the community

Qualifications:

Post-secondary degree in Social Work from an accredited university with current registration with the O.C.S.W.S.S.W; a Registered Nurse or Registered Practical Nurse with current registration with the CNO.; Bachelor’s Degree with a Social Service Worker Diploma and a current registration with the O.C.S.W.S.S.W; or a post-secondary degree with certification with a relevant professional registration (acceptable to the employer).
Experience engaging and working with Indigenous Peoples in a community setting.
Experience and a detailed knowledge of serious mental illness, usually gained by several years of working with this population.
A demonstrated knowledge, awareness and sensitivity to Indigenous Cultures and Traditions.
Valid Ontario Driver’s Licence and access to a vehicle are required.

Hours of Work: 35 hours/week. Flexible hours are required to meet the position requirements.
