About the Opportunity
· We are looking for a Care Coordinator to join the leadership team at Highview Lucan.
· This role is ideal for a nurse who enjoys working with people, developing teams, solving problems, and creating positive experiences for the people they serve.
· You will work closely with the General Manager and leadership team to support clinical services, care planning, medication management, team development, regulatory compliance, and quality improvement throughout the home.
· Unlike many traditional clinical leadership roles, Highview Lucan includes retirement living, Supported Independent Living, and dementia care within one home community. The result is a dynamic and rewarding leadership opportunity that combines strong clinical practice with meaningful relationship-building and team development.

What You'll Be Doing
- Leading and supporting a multidisciplinary care team
- Coaching, mentoring, and developing Associates
- Overseeing care planning, assessments, and clinical documentation
- Supporting medication management and clinical best practices
- Building strong relationships with Residents, Persons, families, and community partners
- Supporting admissions, transitions, and occupancy
- Ensuring compliance with regulatory requirements and organizational standards
- Participating in the rotational leadership on-call schedule
What We're Looking For
- Registered Practical Nurse (RPN) or Registered Nurse (RN) in good standing with the College of Nurses of Ontario
- Experience supporting people living with dementia
- Experience supporting adults with developmental disabilities, autism, and other forms of neurodiversity is considered a strong asset
- Demonstrated leadership experience, including coaching, mentoring, supporting, or leading others
- Strong communication, relationship-building, organizational, and problem-solving skills
- A commitment to person-centred and relationship-centred care
- Valid driver's licence and reliable transportation
