Mobile Crisis Response Team Case Navigator

BACKGROUND:

SCO is a growing and dynamic regional First Nation organization that represents 33 Anishinaabe and Dakota Nations and more than 92,000 citizens in what is now southern Manitoba. Established in 1999 by the Chiefs, SCO is mandated as an independent organization to protect, preserve, promote, and enhance First Nation peoples’ inherent rights, languages, customs, and traditions through the application and implementation of the spirit and intent of the Treaty-making process.

OVERVIEW:

Reporting to the Mental Health and Wellness Program Lead, the Mobile Crisis Response Team (MCRT) Case Navigator will be responsible to manage and coordinate ongoing support, ensure a continuum of care is implemented, and facilitate response efforts for the MCRT to help support crisis intervention, assessment, and ongoing support in mental health and wellness. Integral to this role is the application of a two-eyed seeing approach, ensuring culturally sensitive crisis intervention services are provided seamlessly.

The MCRT Case Navigator will work in a collaborative way with southern Knowledge Keepers, Elders, culturally appropriate mental health teams, and clinical wellness service providers. This position is a crucial in providing immediate and comprehensive support to individuals, families, and communities experiencing crises and substance use challenges.

KEY RESPONSIBILITIES:

Intake and Navigation Services:

· Involvement in Crisis Calls from Beginning to End: Remain actively engaged in all crises calls from the initial incoming call through to the completion of the intake process, team deployment, follow-up care, and final file closure

· Crisis intervention, triage services, and increased capacity for emergency responses to crises

· Community-specific trauma-informed mental health supports to community crisis events, addressing complex and evolving challenges

· Mental Health First Aid screening and assessment to match individuals in crisis to the appropriate level of treatment and support

· Individual client assessments such as suicide assessment and safety plan, self-esteem and depression scale, and mental health status

· Short term one-on-one counselling and referrals to therapy/counselling resources

· Drug and alcohol addiction support triaging using a harm reduction approach

· Grief and loss support

· Domestic and family violence support and triaging

Continuum of Care:

· Identify when a continuum of care is required and be able to match individuals in crisis to the appropriate level of support

· Coordinate with individuals in crisis, MCRT members, and other disciplines to establish a post-crisis continuum of care

· Post-suicide response/aftercare

· Building collaborative partnerships with other organizations to collaboratively enhance the mental health and wellness of First Nations individuals and communities

· Critical incident stress debriefing

Case Management, Documentation, and Reporting:

· Implement a system that keeps the supervisor and team informed about the status of all deployments, including upcoming cases, open cases, cases needing follow-up, and closed cases

· Manage and maintain accurate and up-to-date case records, documentation, and reports regarding all interactions, assessments, and interventions in accordance with policies and procedures

· Ensure compliance with all legal and regulatory requirements

· Facilitate weekly case management meetings

· Maintain accurate and up-to-date information on each crisis call, while also addressing broader system strengths, challenges, and patterns identified during the crisis calls

· When appropriate, organize larger-scale systems meetings to address and support communities facing severe crises or states of emergency

Training and Education:

· Participate in ongoing training and professional development opportunities to enhance knowledge and skills in crisis intervention, culturally appropriate mental health interventions, and trauma-informed care

EDUCATION AND QUALIFICATIONS:

· A degree in a mental health related field and regulated profession such as counselling, social work, therapy, psychology, or psychiatric nursing. Training in Mental Health First Aid for First Nations is an asset

· Two (2) years of experience providing crisis mental health services

· Experience working with southern First Nations and Tribal Councils and their health teams

· Good knowledge of Federal, Provincial and First Nation health care systems including First Nation and Inuit Health Branch, Nursing Stations, and Telehealth technology

· Knowledge of Personal Health Information Act and ability to maintain confidentiality when developing and handling sensitive information

· Team player with a “families first” approach who can work productively and collaboratively with a variety of people within complex systems

· Excellent verbal and written communication skills, with the ability to write clear, concise documents and reports with a high degree of accuracy and for a variety of audiences

· Ability to manage competing deadlines, work independently, and handle confidential or sensitive information appropriately and in an organized fashion

· Proficiency in the use of computers and MS Office Suite (Forms, Word, Excel, Power Point, Outlook)

· Ability to travel to remote communities by vehicle, boat, and/or plane

· Ability to speak Anishinaabe or Dakota languages is considered an asset

OTHER REQUIREMENTS:

· Must provide a current and satisfactory criminal record check with vulnerable sector search, and current and clear Child and Adult Abuse Registry checks

· Must have a valid class 5 MB driver’s license and access to a reliable vehicle as travel will be required

· Must have flexibility for occasional evening, weekend, or overnight meetings or support

· Must be available for on-call outside standard office hours on a rotational basis

· If the employee is a member of a registered profession, they must be a member in good standing with their regulatory body

WAGE RANGE: $65,000 - $80,000 Annually
