Position: Patient Care Navigator
Newleaf Total Wellness Centre seeks a dedicated and compassionate Patient Care Navigator to join our team. This role is pivotal in ensuring that our patients receive the highest standard of care and support as they navigate their treatment plans. The ideal candidate will have a strong background in administration, a thorough understanding of multidisciplinary clinical settings, and education in Medical Office Administration, Business Administration, Kinesiology, Health Sciences, Sports Medicine, Injury Rehabilitation or similar fields.

Key Responsibilities
- Meet with ICBC Patients: Advise patients on their treatment options and assist in booking their appointments.
- Meet with WCBC Patients: Provide clinic orientation and educate patients about the treatment process.
- Support Chronic Pain and Complex Needs Patients: Offer clinic orientation, conduct brief patient needs assessments, initiate care plans, and book necessary appointments.
- Service Choice Consultations: Help patients decide which services best meet their needs. Whether they are dealing with pain, recovering from an injury, or looking to improve their overall wellness, you will help them make informed choices.
- Team Learning & Collaboration: Coordinates team learning and collaboration, supports aligned patient care, and manages patient success stories for marketing..
Qualifications
- Administrative Experience: Proven experience in an administrative role, preferably within a healthcare or multidisciplinary clinic setting.
- Multidisciplinary Clinic Experience: Familiarity with the operations and environment of a multidisciplinary clinic.
- Healthcare Services Knowledge: A strong understanding of various healthcare services and how they support patient care.
- Foundational Biology, Kinesiology, or Human Body Systems Knowledge: Basic knowledge in these areas to better understand patient needs and treatment options.
- Excellent Communication Skills: Superior verbal and written communication skills to effectively interact with patients, caregivers, and the clinical team.
- Critical Thinking Skills: Exceptional problem-solving abilities to efficiently manage patient needs.
