PROGRAM
CONTACT Community Services is seeking a Case Manager to support a Veteran-specific Housing First pilot program in Simcoe County. This role will provide intensive, client centered case management services to Veterans experiencing chronic or episodic homelessness, helping them secure housing, stabilize long term, and improve overall wellbeing. Service delivery will be grounded in the principles of Housing First, with a focus on rapid housing placement, housing stabilization, and long-term wellbeing. The Case Manager will work closely with community partners, Legions, and Veteran specific services to strengthen coordinated care pathways.

MINIMUM REQUIREMENTS OF THIS POSITION INCLUDE:
- Bilingual – English and French is an asset
- Community College Diploma or a University Degree in social services or other closely related field or a combination of education and experience which provides the required knowledge, skills and abilities as determined by the employer
- Minimum of 2 years’ recent full-time work experience within a social services/health care or related environment, including providing case management to individuals/families experiencing chronic/episodic homelessness
- Demonstrated knowledge of Housing First principles, guidelines and related housing concepts, acts and regulations
- Knowledge of psychosocial rehabilitation, recovery principles and harm reduction philosophy
- An understanding of case management; including assessment, service planning, skills reaching, monitoring and evaluation
- Ability to engage sound judgment and strong problem-solving skills; excellent written and verbal communication with administrative and organizational skills and ability to prioritize tasks
- Strong interpersonal skills with proven ability to work independently as well as part of a team
- Demonstrated knowledge of human/legal rights and relevant legislation.
- Full G license and reliable vehicle

DUTIES AND RESPONSIBILITIES (INCLUDING, BUT NOT LIMITED TO):
- Deliver community based case management for Veterans experiencing homelessness, supporting areas such as independent living skills, housing stability, financial management, community participation, employment connections, and social engagement
- Attend community appointments with the individual or provide accompaniment for added support when appropriate
- Support individuals in expanding their opportunities and potential by encouraging participation in social, recreational, educational, and community activities
- Complete thorough assessments to identify the individual’s needs
- Contribute to the creation, execution, and ongoing adjustment of individualized support plans, tracking progress toward established goals
- Provide daily support in a professional, respectful, and discreet manner to help the individual follow their personalized support plan
- Collaborate with the Housing First team to share relevant information that informs the development of individualized support strategies
- Carry out crisis intervention and risk assessments in coordination with a supervisor and/or the Housing First team
- Offer crisis support to individuals, including connecting them with appropriate intervention services
- Advocate for individuals to access essential physical and mental health services, as well as legal and financial resources
- Conduct outreach to potential participants through partnerships with community organizations
