The Opportunity
We are looking for an Opioid Agonist Therapy (OAT) Navigator to join our OAT Team at CUPS on a term basis.
As an OAT Navigator, you will work to engage with clients in various settings to assist with rapid access to life-saving opioid agonist treatment, and other supports. You will provide modelling of healthy recovery habits, support sustained client engagement in the OAT program and will connect participants to CUPS and external agency supports to achieve client goals. You will support clients by providing emotional and social support through dialogue and shared experience, while also building trust and rapport with participants. You will provide safe, quality and client-centered support while reflecting CUPS' shared purpose, ambition, and values.
- Location: Onsite, CUPS - 1001 10 Ave SW, Calgary
- Reports to: OAT Team Lead
- Contract type: Term – Two months
- Schedule: Full-time, Mon to Fri, 40 hours per week
- Starting hourly rate: $26.88 - $28.21 per hour
Key Responsibilities
- Community outreach to healthcare facilities, supported housing and other partner organizations including shelters, engaging participants to provide accurate and relatable information about OAT.
- Build relationships based on trust that will engage participants in the OAT program when they are ready.
- Partner in participants’ recovery by accompanying participants to appointments, navigating supports and advocating on their behalf.
- Engage with hospital teams, engage in discharge planning and facilitate participant connection to CUPS primary care providers.
- Referrals to appropriate resources within CUPS and external resources/partners, including direct referrals and advocacy for participants' admission to treatment/detox facilities.
- Support medication coverage application with participants for both crisis and long-term coverage, support with income, birth certification and identification applications.
- Maintain caseload of regular participants and provide support to colleagues’ caseloads when unavailable do to so due to training or when on leave.
- Assist with research/surveys and completion of Integrated Care Tool with participants.
- Maintaining accurate records and documentation of services within health and other databases as directed.
- Manage participant crisis situations, engaging multidisciplinary team for support and direction, informing partner agencies as needed and appropriate.
- Support and advocate for clients with connections to housing through Coordinated Access and Assessment.
- Transport clients in personal vehicles as needed to create connections to available services, and support client independence to eventually engage with community transportation options.
Employment Requirements:
- Life experience in addiction/mental health/homelessness/low income.
- Valid Driver’s License and reliable vehicle to transport and accompany clients.
- Experience working with individuals/communities experiencing homelessness, issues related to low income, addictions and/or mental health concerns.
