An innovative health program, dedicated to supporting older adults facing homelessness or precarious housing situations compounded by chronic mental health challenges, is seeking a Registered Nurse to play a pivotal role in our interdisciplinary team. This program, designed to provide long-term, dignified support beyond traditional short-term solutions, aims to comprehensively uplift those neglected by the health and social care systems.

Key Responsibilities:

Conduct comprehensive assessments, focusing on physical health, mental health, and substance use.
Collaborate with a multidisciplinary team, including geriatric psychiatrists and outreach counselors, to deliver holistic mental health care and psychosocial support.
Engage in community outreach to support medication management, illness self-management, and effective utilization of health services.

Position Overview:

Full-time role based in Toronto, offering outreach case management services for homeless older adults with complex needs.
Involvement in an innovative program with the support of a dedicated team to find housing solutions, access psychiatric care, and improve psychological and social welfare.

Qualifications:

Current General Class (Practicing) Registration with the College of Nurses of Ontario.
Experience in psychiatric mental health or addiction nursing, particularly in community settings.
Familiarity with administering standardized screening and assessment tools.

Preferred Experience:

Certifications or training in mental health care, gerontology, or working with homeless populations.
A background in crisis intervention, with strong assessment and collaborative care planning skills.
