We are currently looking to hire a:
Registered Practical Nurse
1 – Part-Time, Permanent (17.5 hours per week)
Reporting To: Interprofessional Primary Care Manager
Purpose:
The Scarborough interprofessional primary care access model is a partnership to enhance care for the vulnerable, complex clients living in the Scarborough.
SCHC operates with an integrated and interdisciplinary approach. All roles collaborate with and contribute to different teams and programs to support the community's needs.

Key areas of responsibility:
Clinical:
· Performs specific acts under the College of Nurses Standards of Practice as an RPN.
· Treats minor episodic illnesses as per medical directive and per order.
· Knowledge and experience with venipuncture, ECG testing, immunizations, collecting vital signs and Triaging.
· Conduct health assessments, including history and partial physical, for well clients of all ages.
· Communicate with clients and external providers and services regarding client referrals, test results, follow-up appointments and written prescriptions.
· Participates in chart audits, client case conferences and triage huddles.
· Checks medication samples, orders and maintains vaccines, and returns expired products.
· Provides mentorship and support to volunteers and/or students.
· Perform Triage, Follow up care, home visits.
· Maintain the availability of sterile clinical equipment by autoclaving as necessary.
· Support maintenance of client record systems, including reconciliation of lab results,
correspondence, referral files and computerized registration and encountering.
· Provide Health education and counselling.
· Well women exams (pap, STI testing, breast exams, contraceptive and safe sex education).
· Organizes, implements and evaluates Harm reduction (needle exchange; HIV point of care; safe sex, family planning & safe drug use counselling).
· Well baby exams & vaccinations.
· Postpartum and breastfeeding support.
· Smoking cessation counselling.
· Coordinate stocking of vaccines , POCT, and vaccine fridge monitoring.
· Assist providers with New client intake and with Access clinic.
· Identify needs of clients/community and assists in the development of programs and services as required.
· Maintains complete, accurate and timely records and encounter data in the EMR; participate in chart and case reviews and support client education.
· Participate in training workshops to enhance skills and knowledge.
· Provides mentorship, guidance, and clinical teaching to students, new hires, and learners.
· Other duties as assigned and performed within the scope of the position.
Health Promotion:
· Identify needs of clients/community and assists in the development of programs and services as required.
· Complete outreach and community work through program planning, development, implementation and evaluation.
· Community development and partnership development as required.
Administrative:
· Maintains accurate and complete electronic documentation.
· Participates in inter-professional policy and procedure development.
· Ongoing participation in education workshops to enhance skills and knowledge.
· Participates in resource selection, supports student and volunteer placements.
· Participates in the accreditation process and the implementation of strategic plans.
· Participate in data collection and Quality improvement initiatives.
· Designing and delivering group programs as needed.
· Perform other duties as assigned that are reasonable within the scope of role.
Educational and/or Professional Qualifications:
· Current Certificate of Registration in the General Class with the College of Nurses of Ontario required.
· If graduated prior to 1995, medication administration certificate from an approved community college required.
· Member of RPNAO.
· Venipuncture certification (or willingness to complete).
· Certificate/Diploma in Community Health Nursing considered an asset.
Level of Experience:
· Minimum of two to five years direct patient care.
· Clinical experience within a community based health care setting is an asset.
Skills and Attributes:
· Demonstrated experience/confidence in working independently, with high degree of self-direction and strong organizational skills
· Excellent interpersonal, communication, decision making, problem solving, organizational, conflict management and time management skills
· Oral and/or written fluency in other languages relevant to providing service in the role would be considered an asset
· Familiarity or willingness to practice with electronic health record documentation environment, PS Suite experience considered an asset.
· Ability to work within an Inter-professional model of care.
Sensitivity and awareness of culturally and racially, economically and socially diverse communities
· Willingness to preceptor students from accredited academic facilities
· Current certification in CPR/First Aid or willingness to become certified.
Significant Working Conditions:
· Required to work at different locations to provide coverage as needed
· Flexibility in hours of work to provide extended hours of service including evenings and weekends.
Remuneration: $28.53 - $33.92
