Job Description:
Reporting to the Manager, Community Support Services, the Transportation Scheduler is responsible for supporting the coordination of services and operations within the Transportation Program to meet the needs of individuals in order to promote independent living, increase quality of life and well-being, and ensure optimal level of care and programming in accordance with Carefor’s Mission and Values.

Primary Responsibilities Include:
- Provides program support through such duties as providing program information, receiving and directing client inquiries, and performing coordination duties such as data entry, printing schedules, confirming drives, and supporting coordination coverage as required based on operational need.
- Completes relevant transportation financial duties such as monthly billing reconciliation, verification of driver log sheets, and monitoring outstanding invoices.
- Provides coordination coverage as required based on operational need
- Works collaboratively with other department managers/supervisors/coordinators within a multidisciplinary team and across the organization.
- Completes relevant screening and intake assessments of client utilizing established program procedures and guidelines.
- Describes and documents client’s/family’s responses to care utilizing established procedures
- Liaise with external community partners to ensure effective provision of service delivery such as Health Links, CCTC, Community Mental Health, Regional Hospitals, Regional CSS partners, home and community care partners
- Negotiates with client/family/caregivers during delivery of care and respects client choices and decisions
- Participates in decision making and follows up to promote continuous quality improvement
- Identifies and analyses areas for improvement in work processes and improves these areas with the rest of the team
- Functions in accordance with the Carefor policies, procedures and strategic direction of Carefor
- Functions and reports in compliance with the Occupational Health and Safety legislation, applicable regulations, and Carefor policies and procedures
- Other duties as assigned
Qualifications Include:
- Degree/diploma/certificate in Office Administration or other related field.
- Bilingualism (French/English) an asset
- 3 years’ experience in community healthcare capacity. An acceptable combination of education and experience may also be considered.
- Experience in scheduling and coordinating
- Experience and knowledge of CIMS, Caredove, CHA Screening, an electronic information system is considered an asset
- Experience in statistical and billing reporting
- Knowledge of OHS, Infection Prevention and Control, and WHMIS
- Experience in handling confidential information
- Experience working with clients who have dementia, Alzheimer’s, and mental illnesses an asset
- Experience using information technology systems (Microsoft suite including Excel, scheduling software)
- Highly developed customer service and external relations capabilities.
- Effective communication skills (verbal, written)
- Ability to work in a busy, fast paced environment and prioritize duties
- Interpersonal and interdepartmental relationship building
- Highly developed problem solving and critical thinking abilities
- Ability to work collaboratively as a member of an interdisciplinary team

Salary Range: $40,404.00-$53,040.00 (annual)
Target Hiring Range: $40,404.00-$46,293.00 (annual)
