Compensation: $45,000 - $52,000 per year + Commission + Travel Allowance

Position Summary
The Community Engagement Manager & Jr. Care Planner plays a key role in both client care and business growth. This individual will work closely with the Care Office Owner and office team to support client assessments, coordinate care services, mentor caregivers, develop referral partnerships, and increase awareness of Right at Home throughout Cornwall and Eastern Ontario. The successful candidate will become a trusted resource within the healthcare community while helping families navigate care options and supporting the ongoing growth of the organization.
Key ResponsibilitiesCare Planning & Client Support
- Respond to inquiries from prospective clients and families.
- Participate in client consultations and care assessments.
- Assist in developing and maintaining individualized care plans.
- Conduct client wellness visits and follow-up calls.
- Build strong relationships with clients and family members.
- Support caregiver-client matching and care team assembly.
- Monitor service quality and client satisfaction.
- Work closely with office staff to coordinate care services.
- Maintain accurate client documentation and records.
- Assist with resolving client concerns and service issues.
Team Leadership & Caregiver Support
- Serve as a mentor and resource for PSWs and caregivers.
- Conduct caregiver check-ins and field visits.
- Assist with caregiver recruitment, onboarding and orientation.
- Promote quality care and service excellence.
- Help foster a positive and supportive team culture.
Community Engagement & Business Development
- Build relationships with hospitals, retirement residences, physicians, occupational therapists, physiotherapists, social workers, discharge planners, case managers, and community organizations.
- Conduct regular outreach and networking activities throughout Cornwall and Eastern Ontario.
- Represent Right at Home at community events, trade shows, health fairs, seniors' events, and networking functions.
- Deliver Lunch & Learns and educational presentations.
- Identify and develop new referral partnerships.
- Support local marketing initiatives and community awareness campaigns.
- Track outreach activities and referral opportunities.
- Assist in growing Right at Home's presence throughout Eastern Ontario.
TerritoryThis role will support business development and client care activities throughout:
- Cornwall
- SDG Counties
- Prescott-Russell
- Brockville
- Leeds & Grenville
- Other Eastern Ontario communities as required
Travel throughout the territory is expected.
Qualifications
- Experience in healthcare, home care, community services, sales, customer service, business development, care coordination, or related fields.
- PSW, Social Service Worker, Healthcare Administration, Nursing, Business, or related education considered an asset.
- Strong relationship-building and networking skills.
- Excellent verbal and written communication abilities.
- Comfortable presenting to groups and community organizations.
- Highly organized and self-motivated.
- Strong problem-solving and customer service skills.
- Proficient with Microsoft Office and technology platforms.
- Valid Ontario Driver's License and reliable vehicle required.
- Current CPR/First Aid certification considered an asset.
- Vulnerable Sector Check required.
Compensation & Incentives
- Base Salary: $45,000 - $52,000 annually
- Commission on private-pay clients
- Monthly travel allowance
- Mileage reimbursement for approved business travel
