Outreach Worker - General
Wraparound/Supervised Access Programs
Start Date: As soon as available
POSITION SUMMARY: Provides guidance and support to children, youth, and their families with strengths-based/trauma-informed interventions that follow a psycho-educational model. Provides positive parenting support and/or skills in the areas of communication, coping, independent living, and social/emotional/regulation in order to reduce harm or risk factors that participants may be experiencing.
This position is in the Wraparound and Supportive Visitation Programs.
Wage: $25.50/hr (plus benefits after completion of probation)
Hours: 22 hrs/wk
QUALIFICATIONS:
- Secured local housing
- Post-secondary diploma/certificate in relevant field and experience supporting children, youth, and families.
- Training in child/adolescent development, trauma-informed interventions, collaborative problem solving preferred
- Satisfactory criminal record check
- Valid BC driver’s license, reliable vehicle, and clear Driver’s Abstract
- Chosen candidate must pre-arrange housing in Creston, BC if moving for position
OUTREACH WORKER JOB RESPONSIBILITIES:
- Screen referrals when VCS screener is unavailable
- Maintain files as per program manual/organization requirements
- Provide services in a collaborative manner with MCFD and/or community service providers using an Integrated Case management practice as the framework
- Provide necessary emotional/educational support to children, youth, and their families through trauma-informed/strengths-based/ solution-focused, psycho-educational interventions
- Participate in regular meetings and consult with Outreach Supervisor or designate
- In emergencies, contact Outreach Supervisor, Program Manager, Executive Director, Social Worker and/or Helpline for Children as appropriate
- Provide service to families on a flexible schedule, at times and locations that are convenient to the family served
- In collaboration with the participant, develop a service plan that outlines strengths, needs, abilities, preferences, goals/timeframes of service
- Provide discharge summaries within two weeks after closing the service
- Provide monthly written reports outlining goals, strategies, and progress of service (mandated service only)
- Provide updates as required to the appropriate social worker (mandated service only) and to the Outreach Supervisor at least monthly
- Review goals with service recipients every 3 months
- Take part in professional development
PROGRAM SPECIFIC JOB RESPONSIBILITIES (IN ADDITION TO ABOVE):
WRAPAROUND:
- Provide short-term intensive support and interpersonal skill enhancement services to families to reduce or prevent child risk as per contract
- Provide parents with the opportunity to develop and display new skills through interaction with children
- Provide services for parents, children, and youth who need to obtain skills such as budgeting, shopping, household management, social skills, employment/housing search, etc. to reduce harm or risk factors they or their children may be experiencing
- Assess basic life skill levels
- Help participants make a transition from adolescence to young adulthood, when appropriate
- Assist participants to effectively communicate their needs and concerns in a socially appropriate manner
- Assist participants to develop coping strategies within their family, school, workplace and community
- Assist and teach participants how to access appropriate community services
SUPPORTIVE VISITATION/SUPERVISED ACCESS:
- Provide a forum for safe and monitored access between children/youth in MCFD care and their parents and relatives
- Ensure the emotional and physical safety of children during visits with family
- Address the impacts of separation and loss on family members
- At or before 1st meeting, collaborate with the parent and/or family and social worker to develop and sign a supervised visitation plan, to clarify roles/responsibilities, arrange visitation time and location, and confirm the transportation schedule
- Conduct supervised visitation sessions as scheduled
- Provide intensive support and interpersonal skill enhancement services to families to reduce child risk as per contract
- Provide parents with the opportunity to develop and display new skills through interaction with children
