Spiritual Care Practitioner
Unity Health Toronto is seeking a permanent part-time (0.5 FTE) Spiritual Care Practitioner to join the Providence Healthcare site. Clinical assignments will be based on operational and site needs. Reporting to the Senior Manager, Mission, Values & Spiritual Care, the successful candidate will join a high-performing, clinically integrated team providing person-centred, culturally appropriate spiritual care that honours spiritual diversity for patients, residents, and their families.
Days of Work : Mondays, Fridays & every other Wednesday
Hours of Work : 9:00 am – 5:00 pm
DUTIES & RESPONSIBILITIES :
Provide and promote high-quality spiritual, religious, and cultural care for patients, families, and staff, consistent with the Mission and Values of Unity Health Toronto.
Assess, plan, implement, and evaluate the spiritual care needs of patients and their families, and communicate spiritual assessments and care plans clearly to the interprofessional team through concise and timely documentation.
Provide care that is respectful of religious, spiritual, and cultural diversity, supporting patients, families, and staff in ethical deliberation and complex decision-making.
Engage patients, families, and staff in understanding the interconnected dimensions of spiritual, religious, physical, emotional, psychological, and mental well-being.
Provide appropriate opportunities for prayer, sacraments, and other spiritual, religious, or cultural rituals.
Be available and accessible to patients, families, and staff across the continuum of care, particularly those experiencing spiritual and emotional distress.
Demonstrate best practices within rehabilitation, end-of-life care, and grief and bereavement support.
Enable effective team collaboration and actively participate as a full member of diverse clinical teams.
Facilitate a variety of staff support strategies that foster collaboration and a supportive community.
Collaborate with community clergy and spiritual care partners, where appropriate, and facilitate referrals and connections to patients’ and families’ community supports.
QUALIFICATIONS :
Successful completion of Clinical Psycho-Spiritual Education (CPE)
Employees must maintain membership with the Canadian Association for Spiritual Care (CASC), or one of its partner Spiritual Care professional organizations.
Masters Degree in Divinity, Psycho-Spiritual Studies, Theology, or equivalent.
Ability to integrate theology and behavioural science into the practice of spiritual care.
Demonstrated competency in providing spiritual care to individuals experiencing illness and to persons from diverse religious, spiritual, cultural and educational backgrounds, facilitating the exploration of meaning, purpose, and personal values.
Demonstrated knowledge and experience in rehabilitation, palliative and end-of-life care, as well as the care of older adults and individuals with cognitive impairment.
Previous clinical experience working with patients/families experiencing trauma, illness, and disadvantage.
Demonstrated spiritual maturity and integrity complimented by sensitivity, compassion, humility, gentleness, approachability, and a genuine commitment to caring for others.
Demonstrated knowledge of a range of spiritual care models and the ability to apply them appropriately across diverse practice settings.
Demonstrated conflict resolution skills and crisis intervention techniques.
Demonstrated ability to collaborate effectively as a member of both the Spiritual Care and inter-professional teams while also working independently with initiative and sound judgment.
Excellent communication (verbal/written English), interpersonal, organizational and problem-solving skills are essential.
Demonstrated proficiency working with computer applications in a Windows environment (Word, Electronic mail, electronic documentation).
Demonstrated commitment to the Mission and Values of Unity Health Toronto.
Must have an excellent attendance record.
