Job Overview
The Family Intervention Intake Coordinator is responsible for coordinating the intake and referral process for families accessing family intervention services. The position serves as the first point of contact for families, referral agencies, community partners, and service providers. The Intake Coordinator gathers and documents information, assesses initial service needs, determines appropriate referrals and supports, coordinates intake appointments, and ensures families are connected with appropriate programs and resources. This position works from a family-centered, culturally responsive, trauma-informed, strengths-based, and Indigenous-informed approach, recognizing the importance of family, community, culture, identity, and traditional supports. The Intake Coordinator works collaboratively with Family Intervention staff, Children Services, Indigenous organizations, and other partnerships to support timely and coordinated services for children, youth, and families.
Responsibilities
Cultural and Community Responsibilities
- Incorporate indigenous cultural values, traditions, practices, and approaches into service coordination.
- Support culturally safe and respectful service delivery
- Encourage connection to elders, knowledge keepers, cultural programs, community supports, and traditional resources
- Demonstrate respect for indigenous families, cultures, histories, languages, traditions, and diverse family structures.
- Participate in cultural learning and professional development opportunities.
- Recognize and respect indigenous family structures, traditional knowledge, cultural practices, and community connections.
Intake and Referral Coordination
- Receive and respond to referrals through telephone, email, in-person contact, and other approved referral processes.
- Conduct initial intake interviews with families and referring agencies such as Children Services.
- Gather relevant information regarding family circumstances, presenting concerns, strengths, needs, and requested support.
- Complete intake forms, screening tools, consent documentation, and other required assessments.
- Identify immediate concerns, service needs, and potential safety issues and bring them forward to the appropriate supervisor or staff member.
- Determine appropriate program eligibility and service pathways in accordance with organizational policies and procedures.
- Coordinate intake appointments and ensure families receive information about available services.
- Provide appropriate referrals to internal programs, community resources, and external service providers.
- Maintain a current resource and referral directory.
Administrative Responsibilities
- Maintain organized intake files and records
- Track program statistics and data intake
- Assist with program reports, audits, and funding requirements
- Monitor waitlists and service capacity
- Assist with scheduling, correspondence, meeting coordination, and other administrative duties
- Maintain confidentiality and professional boundaries at all times
Case Coordination and Documentation
- Maintain accurate, timely, and confidential client records
- Enter intake and referral information into approved databases and case-management systems.
- Track referrals, appointments, waitlists, service connection, and follow-up activities
- Ensure documentation complies with organizational policies, privacy requirements, funding requirements, and applicable legislation
- Monitor outstanding referrals and ensure appropriate follow-up occurs.
Collaboration and Communication
- Work closely with Family Intervention Support Workers, Supervisors, Managers, and Children Services
- Participate in team meetings
- Communicate professionally with external agencies and community partners
- Develop and maintain positive working relationships with indigenous communities, organizations, and service providers
- Coordinate referrals and information sharing while ensuring appropriate consent and confidentiality.
Requirements
- Satisfactory Police Information Check and Intervention Record Check.
- Diploma or degree in Social Work, Human Services, Indigenous Studies, Community Services, or a related field is preferred
- Equivalent education and experience may be considered.
- Previous experience in intake, family services, child and family intervention, case management, community services, or a related field is preferred.
- Experience working with indigenous families and communities is considered a strong asset.
- Strong interpersonal and communication skills
- Excellent organization, time management, and documentation skills
- Ability to conduct sensitive intake conversations with compassion and professionalism
- Ability to prioritize competing demands and respond appropriately to urgent situations
- Knowledge of trauma informed and strength-based practices.
- Understanding of family systems and the impact of social, economic, cultural, and historical factors on families
- Ability to work respectfully with people from diverse backgrounds
- Proficiency with computers, electronic records, email, Microsoft Office, and database systems
Pay: $27.00-$32.00 per hour
