What You'll Do
As our Community Nurse Manager (Branch Lead), you will:
- Lead, coach, and inspire a team of nurses and personal support workers.
- Conduct client consultations, nursing assessments/reassessments, supervisory visits, and quality reviews.
- Guide families through complex healthcare decisions by understanding their goals, challenges, and existing supports, then developing thoughtful, person-centred care plans.
- Build trusted relationships with clients, families, hospitals, physicians, discharge planners and community partners.
- Foster employee engagement, accountability, and a positive team culture.
- Drive quality improvement initiatives and support branch performance.
- Develop, review and update individualized care plans.
- Medication management, including Best Possible Medication History (BPMH) and medication reconciliation
- Review remote monitoring alerts and act as needed
- Provide direct client care when needed to support your team and ensure continuity of care.
- Identify opportunities to improve client outcomes, strengthen partnerships, and support the continued growth of the branch.
Who Thrives in This Role?
We're looking for someone who:
- Is a Registered Nurse (RN) or Registered Practical Nurse (RPN) in good standing with the College of Nurses of Ontario.
- Has developed strong clinical judgment and critical thinking through hands-on nursing experience.
- Naturally builds trust and meaningful relationships.
- Has at least three years of nursing experience and is ready to take the next step in the leadership journey.
- Enjoys mentoring and developing others.
- Thinks beyond the immediate problem and seeks practical, long-term solutions.
- Is proactive, curious, and excited by new challenges.
- Leads with compassion, integrity, and accountability.
- Holds a valid Ontario driver's licence and has access to a reliable vehicle.
