Position Overview: As a Nurse Practitioner at Durand, you will play a vital role in delivering high-quality, patient-centred medical care for our diverse clientele. The successful candidate will provide comprehensive medical services, including diagnosis, treatment, and preventative care, while ensuring an exceptional patient experience. You will work with our physicians, RPN, and multidisciplinary paramedical team to assess, diagnose, and treat a diverse patient population, ensuring the highest standards of care. This role requires a high level of comfort in managing patients both in-person and virtually, as appropriate in a fact-paced environment.
Responsibilities:
Clinical Assessment and Diagnosis:
- Conduct thorough patient assessments, including medical history reviews and physical examinations
- Formulate accurate diagnoses and develop appropriate treatment plans
- Diagnose acute and chronic conditions, develop individualized treatment plans, and prescribe medications in accordance with provincial regulations
- Perform diagnostic tests, including ECGs (EKGs) and other basic medical assessments.
- Monitor and record patient vitals and conduct health screenings
Treatment and Management:
- Prescribe medications and treatments according to provincial regulations and facility protocols
- Perform IV therapy and oversee IV administration
- Wound care and assist with minor procedures
- Provide primary and family medical care, including for pediatric patients.
- Conduct routine follow-ups and monitor patient progress to adjust treatments as necessary
- Administer vaccines and perform phlebotomy as required
- Utilize EMR systems to maintain accurate and detailed patient records
- Provide mental health support and services as needed
Collaborative Care:
- Work closely with physicians, nurses, and other healthcare professionals to coordinate patient care
- Share insights and findings to ensure comprehensive care
- Participate in case conferences and contribute to collaborative decision-making
- Ensure plans are aligned with overall care objectives, providing a cohesive approach to patient health.
- Formulate treatment strategies in conjunction with family physicians
Patient Advocacy:
- Coordinate out-patient referrals for diagnostic tests and specialists
- Collaborate with patients in developing and implementing care plans
- Provide patient education on treatment plans, medications and healthy lifestyle choices
Qualifications:
- Current Certification of Registration with the College of Nurses of Ontario and in good standing
- Master's degree in Nursing with a provincially approved college
- 3-5 Years of clinical experience as a Nurse Practitioner working in primary care or family medicine with pediatrics is an asset
- Experience and efficiency in prescribing medications
- Strong ability to diagnose and treat a wide range of medical conditions
- Strong clinical decision-making skills
- Vaccinations, phlebotomy, and IV therapy is highly valued
- Exceptional communication, accurate documentation including experience with EMR system and interpersonal skills
- Proficiency in both in-person and virtual health consultations
- Exceptional clinical and leadership skills
- Current CPR and first aid certification
- Professional liability protection
- Commitment to providing patient-centered and evidence-based care
- Ultrasound, EKG/ECG diagnostic testing experience an asset

Pay: $41.26-$98.74 per hour
