Job Summary
The Registered Dietitian will be a member of the Interprofessional Primary Care Team and provide client-centered, evidence-informed nutrition services to individuals, families, and caregivers receiving care through the Primary Care Clinic and community-based programs. The Dietitian works collaboratively with clients and the interdisciplinary team to assess nutritional needs, develop individualized care plans, support chronic disease prevention and management, and promote overall health and well-being. Services are delivered through individual nutrition counseling, group education, health promotion initiatives, and community outreach. The Dietitian develops and facilitates workshops on a variety of topics, including healthy eating, prenatal and postnatal nutrition, infant feeding, and chronic disease management. The role is committed to advancing health equity by improving access to nutrition services for culturally diverse populations and individuals experiencing barriers to care.
Job Specific Responsibilities:
- Conduct comprehensive nutrition assessments and provide individualized nutrition counseling, education, intervention planning, and ongoing support for clients and caregivers, promoting health, well-being, and chronic disease prevention and management.
- Develop, coordinate, and facilitate group education sessions, workshops, and community outreach initiatives on topics such as healthy eating, prenatal and postnatal nutrition, infant feeding, and chronic disease management, with a focus on improving access for diverse and underserved populations.
- Participate in interdisciplinary case consultations, care conferences, team meetings, and information-sharing activities, contributing nutrition expertise to collaborative care planning and integrated service delivery.
- Develop program content, educational materials, and service delivery plans in collaboration with team members, ensuring resources are culturally appropriate and available in relevant languages.
- Refer individuals and groups to appropriate internal and external services, programs, and community resources.
- Provide consultation, education, and support to primary care staff and other interdisciplinary team members on nutrition-related matters.
- Collaborate with internal and external service providers, community agencies, and partner organizations to support coordinated client care and health promotion initiatives.
- Ensure participant feedback and program evaluation activities are completed and submitted in accordance with program requirements.
- Maintain accurate documentation, records, and reporting requirements in accordance with professional standards and organizational policies.
- Perform other related duties as assigned.

- Contributing to the Centre's efforts to secure and maximize resources for current and new programs, services and activities.
- Participating in the Centre's efforts to enhance its capacity through staff development.
- Working in a manner that supports the Centre's Occupational Health and Safety policies and procedures.
Qualifications:
- Current (and in good standing) Registration with the College of the Ontario College of Dietitians
- Three to five years experience working within a community-based environment.
- Demonstrated experience or strong interest in working with one or more of PQCHC's priority populations: newcomers and refugees, older adults, 2SLGBTQ+ and gender-diverse clients, and people experiencing health inequities linked to the social determinants of health, such as income, housing, and employment.
- Demonstrated commitment to and knowledge of community-based health care.
- Technology proficiency an asset, PS Suite EMR, Microsoft Office Suite.

- Linguistic profile: Bilingual English/French is considered a strong asset
