The City of Kawartha Lakes Family Health Team (CKLFHT) is currently seeking a permanent, full-time Nurse Practitioner to join our team of health care professionals.
Nurse Practitioners are a key part of the patient care circle and should be comfortable working to their full scope of practice. You'll collaborate closely with physicians and CKLFHT colleagues to deliver comprehensive clinical services to children and adults — spanning direct patient care, program development, and community linkages. This is a primarily in-person role based at one of our CKLFHT locations within Kawartha Lakes.
What you'll do
- Diagnose and treat acute and episodic illness within an interprofessional team model
- Deliver health promotion, disease prevention, and chronic disease management services
- Coordinate patient care plans in consultation with physicians, the care team, hospitals, community clinics, and community partners
- Advocate for patients, provide mentorship, and serve as an educational resource as needed
- Collect and contribute data for statistical and reporting requirements
- Uphold and embody the values and philosophy of the Family Health Team
Qualifications
- Completion of a recognized Primary Care Nurse Practitioner program (NP-PHC, Extended Class)
- Current registration with the College of Nurses of Ontario (CNO), unrestricted
- Active membership in good standing with RNAO/NPAO, including professional liability coverage
- Master of Nursing degree is considered an asset
- Minimum 2 years of experience, preferably in a primary care setting
- Current CPR certification (maintained throughout employment)
Skills & abilities
- Independent clinical assessment and patient management skills
- Strong grounding in evidence-based primary care practices, with emphasis on health promotion and risk reduction
- Proven ability to collaborate effectively with interprofessional teams, patients, physicians, and community partners
- Strong organizational, communication, and problem-solving skills with high attention to detail
- Comfortable managing competing priorities in a dynamic care environment
- Proficiency in Microsoft Word, Excel, and Outlook; experience with an EMR (Telus/PS Suites preferred)
Additional requirements
- Current membership in good standing with CNO, RNAO/NPAO, and applicable regulatory body; proof of professional liability insurance
- Satisfactory Criminal Record and Judicial Matters check, including Vulnerable Sector screening
- Proof of current immunization per the Canadian Immunization Guide for Healthcare Workers
- Eligible to work in Canada; valid driver's license and regular access to a vehicle for community travel
Annual Salary: $110,265 - $129,193
