Eli's Place is opening Canada's first long-term residential therapeutic community for young adults living with serious mental illness, on a 600-acre working farm north of Guelph. We're building a founding clinical team, and the Nurse Practitioner is the primary health care provider at the heart of the community.
The Nurse Practitioner practises autonomously within the Nurse Practitioner scope of practice as a core member of the interdisciplinary clinical team. You'll provide primary and mental health care to Guests from within the daily life of the community, not from a removed position, working closely with the Clinical Director, Clinicians, the consulting Psychiatrist and Guest services staff. As a founding hire, you'll help establish the admission health assessment process, medication management practices and health and safety protocols that define primary care at Eli's Place.
The role reports to the Clinical Director. It's full-time and on site, and because care here is delivered alongside Guests it can't be done remotely. The schedule follows clinical and program needs.
What you'll do
- Provide primary health care to Guests, including assessment, diagnosis and treatment within the Nurse Practitioner scope of practice
- Conduct health assessments at admission and monitor physical and mental health throughout each Guest's stay
- Manage medication prescribing and monitoring in collaboration with the consulting Psychiatrist
- Conduct risk assessments, including suicide and self-harm risk, and respond to changes in risk
- Support the clinical team in treatment planning, progress review and care coordination, and contribute to admissions screening
- Maintain accurate clinical documentation, and help establish primary care practices, health and safety protocols and emergency procedures through pre-launch and early operations
What we're looking for
- A Master's degree in Nursing with Nurse Practitioner designation
- Registration in good standing with the College of Nurses of Ontario
- Current professional liability protection appropriate to the role
- At least two years of experience in mental health, addictions or community health settings
- Experience working with people living with serious mental illness is strongly preferred
- Experience with co-occurring mental health and substance use presentations is an asset
- Confident autonomous practice, with sound clinical judgment and a calm presence in complex or crisis situations
- Proficiency with electronic health record systems
- Strong communication with Guests, families and the clinical team
Compensation and benefits
The salary range is $122,000 to $145,000, based on experience and qualifications. Eli's Place offers a competitive health and wellness benefits package, three weeks of vacation and a dedicated professional development fund. College registration fees are paid by the organization where registration is a condition of employment.
This is a small founding team building something that has never existed in Canada, on a working farm where the daily environment is unlike anything an institutional or urban clinical setting can offer. For the right person, that context is as much a part of the offer as the salary.
