Case Manager – Supportive Housing

Position Summary
The Case Manager at Rising Tide plays a pivotal role in supporting individuals facing homelessness as they transition into stable housing. This position involves working directly with clients to develop and implement personalized case plans that address their unique needs and challenges, including mental health, polysubstance use, and other barriers to stability.
This role requires flexibility, resilience, and a strong commitment to Rising Tide’s mission and values, ensuring all interactions are grounded in respect, empathy, and professionalism.
The Case Manager is responsible for documenting all client interactions and progress in the Homeless Individuals and Families Information System (HIFIS) and Rising Tide’s internal systems, as required, and reports directly to the Manager of Supportive Housing.
Applicants must have an interest and ability to work with individuals from diverse racial, ethnic, and socio-economic backgrounds who are experiencing homelessness, mental health challenges, and/or substance use disorders.
Key Responsibilities
Case Planning and Client Support
- Collaborate with clients to create individualized case plans outlining goals, strategies, and steps toward achieving housing stability and personal growth.
- Regularly assess clients’ progress and adjust case plans to meet evolving needs.
- Conduct home and community visits to monitor client progress, provide support, and build strong, trusting relationships.
- Apply critical thinking to assess clients’ behavior, mental health, and polysubstance use to guide case management decisions.
Coordination and Collaboration
- Work closely with community partners, including social services, healthcare providers, addiction services, and government agencies, to coordinate comprehensive client support.
- Act as a liaison between clients and external resources, advocating for clients’ needs and ensuring access to services.
- Participate in multidisciplinary team meetings with Rising Tide staff, including Housing Support Workers and Peer Support Workers.
Crisis Management and Intervention
- Respond to crisis situations, including mental health emergencies, substance use incidents, and other urgent client needs.
- Utilize non-violent crisis intervention techniques and trauma-informed approaches to de-escalate situations effectively.
Documentation and Compliance
- Maintain accurate and up-to-date records of client interactions, assessments, case plans, and progress notes in HIFIS.
- Ensure confidentiality and privacy of all client information according to legal and organizational standards.
Application of Best Practices
- Apply harm reduction techniques when supporting clients experiencing polysubstance use.
- Employ trauma-informed care principles in all interactions.
- Demonstrate cultural competency and sensitivity, ensuring services are accessible and respectful of clients’ diverse backgrounds.
Independent and Team Work
- Manage an independent caseload, demonstrating strong organizational skills and self-motivation.
- Collaborate effectively with the broader Rising Tide team to enhance client outcomes.
- Flexibly adjust work hours, including evenings and weekends, as required to meet client needs.
- Perform other duties as assigned.
Education and Experience
- Bachelor of Social Work, Human Services Counsellor Diploma, or equivalent.
- Minimum of 2 years’ experience in a frontline case management role serving adult vulnerable populations.
- A combination of education, training, and relevant experience will also be considered.
- Strong interpersonal, communication, and conflict-resolution skills.
- Ability to work independently and collaboratively within a multidisciplinary team.
- Knowledge of community services, harm reduction, trauma-informed care, and culturally sensitive practices.
Training and Certifications
- Certification in Non-Violent Crisis Intervention and Applied Suicide Intervention Skills Training (ASIST) required (or willingness to obtain at earliest opportunity).
- First Aid and CPR certification (or willingness to obtain).
- Proficiency in computer applications including email, Microsoft Office, and HIFIS.
Additional Requirements
- Flexibility to work varied hours, including evenings and weekends, as required.
- Valid driver’s license and access to a reliable vehicle, as the role may involve travel in the Greater Moncton Area.
