Position Summary
Working within the context of a culturally safe, client-centered focus, the Home and Community Care Nurse will be responsible to assess, plan, organize, implement, and evaluate professional nursing care for clients with acute, chronic, or palliative health care needs in a client’s home, clinic and/or community setting. This client-centered care emphasizes the promotion, maintenance, and restoration of holistic health, and the prevention and treatment of disease through teaching, counseling, and direct nursing care in a culturally respectful and safe manner. The Home and Community Care Nurse participates in health promotion activities and acts as a liaison between the program, hospital units, community health providers, and community support services.
Roles and Responsibilities
- Provides care and service that meets the standards for Licensed Practical Nurses, Registered Nurses, and Registered Psychiatric Nurses within the BC College of Nurses and Midwives (BCCNM). Works within scope of practice for their license as determined by relevant legislation, BCCNM standards, organizational policy, and individual clinical competency. Seeks guidance from BCCNM and/or FNHA Regional Practice Support when required.
- Provides culturally safe care using a trauma-informed lens and acts as an advocate for services that promote and protect the health and wellbeing of individuals, families, and communities.
- Provides holistic nursing services to clients in their homes, community, and/or clinic setting by assessing physical, emotional, spiritual, mental, and psychosocial needs of clients and their families.
- Understands the aging population’s medical needs, their understanding of health care, and their cultural beliefs when working closely with Elders.
- Applies and utilizes nursing knowledge and critical thinking to choose options and to plan, implement, and evaluate interventions to address health issues.
- Provides clients and family caregivers with health information as required through demonstrations and informal teaching sessions.
- Works closely with interdisciplinary team members within the Health Centre and other departments within the Nation and communicates effectively while contributing to a collaborative and team-oriented work environment.
- Assists interdisciplinary team members with organizing and hosting Elders’ programs, lunches, cultural gatherings, community events, and wellness activities, including providing nursing support, health oversight, advocacy, and assistance to Elders participating in these activities as required.
- Takes a leadership role in the Home Care Program, including coordinating and maintaining client service schedules, adjusting services and staffing as required, and ensuring continuity of care when changes occur.
- Provides direct supervision, leadership, guidance, and support to Home Support Workers and assists in establishing and monitoring performance expectations
- Assists clients and families with navigating and understanding Blue Cross and First Nations Health Authority (FNHA) Health Benefits.
- Collaborates with clients in formulating short- and long-term goals; coordinates a multidisciplinary care plan; evaluates the care plan to ensure continuity of care; and discharges individuals where and when appropriate.
- Seeks out and participates in relevant continuing professional education opportunities to maintain current and competent practice.
- Coordinates home care services by liaising with clients, families, Home Support Workers, health care providers, hospitals, community agencies, and allied health professionals. Coordinates discharge planning for clients returning home from hospital, arranges home support services, and facilitates access to external resources and support services to ensure client needs are met.
- Works closely and effectively with other health authorities and service providers, including Island Health and the First Nations Health Authority.
- Demonstrates flexibility and a willingness to support organizational programs, initiatives, and operational needs, as required, while working within the scope of practice, qualifications, and responsibilities of the position.
- Performs other related duties as assigned, consistent with the scope, qualifications, and responsibilities of the position.

Qualifications:
- Graduate of an approved Practical Nursing program recognized in British Columbia.
- Current practicing registration and member in good standing with the BC College of Nurses and Midwives (BCCNM) as a Licensed Practical Nurse (LPN)
- Minimum two (2) to three (3) years of recent experience in home and community care, public health, community health, primary care, or a related nursing field.
- Current CPR and First Aid certification.
- Satisfactory Criminal Record Check.
- Valid Class 5 Driver's License and access to a reliable vehicle for travel within the community and surrounding service areas.

Additional Preferred Qualifications/Certifications
- Completion of immunization certification within one year.
- Foot care certification.
- Transportation of Dangerous Goods certification.
- San’yas Indigenous Cultural Safety Training.
- Experience working in a First Nations community preferred.

Knowledge, Skills and Abilities
- Knowledge of, and ability to apply, an understanding of First Nations cultural principles and protocols in work situations and respect those cultures free of discrimination or racism.
- Ability to communicate effectively with First Nations people, families, and community members to establish rapport.
- Understanding of historical impacts and long-term trauma effects on First Nations people, specifically residential school impacts and intergenerational trauma.
- Knowledge of First Nations Health Authority and the services they provide.
- Knowledge of health status of populations, inequities in health, determinants of health and illness, principles of primary care, strategies for health promotion, disease and injury prevention, health protection, curative, urgent and emergent care, rehabilitation, and supportive or palliative care.
- Demonstrated ability to teach clients, families, community caregivers, Home Support Workers, and other staff on health care issues.
- Knowledge of health care services, community resources, agencies, and their roles and responsibilities in providing a continuum of care.
- Ability to coordinate services and collaborate effectively with clients, families, community partners, and multidisciplinary care providers.
- Ability to work independently with minimal supervision.
- Physical ability to perform the duties of the position.

Schedule and Compensation
Shifts: 3 days/week, 9:00 a.m. – 4:30 p.m.
This position is opened until filled
Wage: Dependent on experience.
Pay: $33.00-$38.00 per hour
