Care Coordinator (Full-Time)

Annapolis Valley Home Care is a locally owned and operated home care agency proudly serving communities throughout the Annapolis Valley. With a team of more than 100 Caregivers and a dedicated office staff, we are committed to providing compassionate, high-quality care to our clients while fostering a supportive workplace for our employees.

Due to continued growth, we are seeking a full-time Care Coordinator to join our office team.

Position Summary

The Care Coordinator plays a key role in the day-to-day operations of our agency by coordinating client services, supporting our Caregiver team, and ensuring exceptional customer service. This is a fast-paced, rewarding position that requires strong organizational skills, excellent communication, and the ability to manage multiple priorities.

Responsibilities

Responsibilities include, but are not limited to:

Scheduling Caregivers and coordinating client services
Conducting client consultations and home visits
Building and maintaining positive relationships with clients and their families
Coordinating Caregiver orientations and ongoing training workshops
Assisting with recruitment, interviewing, hiring, and reference checks
Maintaining accurate employee and client documentation
Working collaboratively with the office team to ensure continuity of care
Participating in the paid after-hours emergency phone rotation (approximately one evening per week and every fifth weekend)
Providing exceptional customer service while responding to changing client and staffing needs

Qualifications

Preferred qualifications include:

CCA Certification or equivalent education and/or relevant experience in healthcare or home care
Previous scheduling or administrative experience
Experience in human resources or recruitment is considered an asset
Excellent verbal and written communication skills
Strong organizational, time management, and problem-solving abilities
Ability to multitask and work effectively in a fast-paced environment
Proficiency with Microsoft Office and general computer applications
Valid driver's license and reliable transportation
Clean Criminal Record Check and Vulnerable Sector Check

Compensation & Benefits

Full-time salaried position based on 75 hours bi-weekly
Equivalent hourly salary of $25.00–$27.00, based on qualifications and experience
Company-paid health benefits

One week of paid sick/medical leave annually

Schedule

Office Hours

Monday to Friday
8:00 a.m. – 4:00 p.m.

The workday includes one paid 15-minute break and one 30-minute lunch break.
