Position Summary:

The Canadian Mental Health Association, Simcoe County Branch seeks a Social Worker, who, in accordance with the agency’s policies, standards, and guidelines, is responsible for providing care/service to clients in areas related to: outreach, individualized assessment & planning, direct service provision, coordination & support, monitoring & evaluation, and advocacy.

What You'll Do:

Provide leadership to ACT Team as to how they should engage client families and natural supports in the treatment/service planning process, as well as in individual and/or multiple family support/therapy.
Perform as a member of a critically essential program with other multidisciplinary clinicians.
Perform general duties as specified within the ACT Team model of care.
Provide the team, client, family, and others with specialized consultation/education, related to social work activities.
Participates in a multidisciplinary consultive, client-centred recovery orientated specialized mental health treatment/medical service delivery model.
Provides the majority of the treatment, rehabilitation and support services, in addition to clinical & psychosocial rehabilitation and recovery for persons who have the most serious mental illness, have severe symptoms and functional impairments, and have not benefited from traditional outpatient programs.
Provides therapeutic counselling interventions, psychosocial assessments and family support.
Develop, implement, and document The Ontario Common Assessment of Need (OCAN)/Recovery Plans in conjunction with the client and other ACT Team members.
Create and facilitate therapeutic and skill building groups.
Provide counselling to assist client in accomplishing their goals and managing their symptoms/day to day stressors.
Provide direct service to clients (both individually and in groups) as per agency guidelines, policies, and procedures, including: skill teaching on activities and behaviours of daily living, assistance in obtaining and maintaining housing, crisis intervention strategies/support, relapse prevention strategies, self-care strategies/support, and family support/education, as appropriate for supporting the interests of the client.
Provide on-call crisis intervention to ACT Team clients during regular work hours.
Provide information and strategies for health promotion.
Be adaptive as the ACT team is mobile and delivers majority of treatment, rehabilitation, and assertive outreach in the community (75% or more of the services are provided outside of the program offices, in community).
Documents assessment and interventions as they relate to the client’s mental illness symptoms and behaviors and response to medication and shall monitor for medication side effects. Reports comprehensive assessments to team psychiatrist.
Accountable and timely reporting to appropriate tam clinicals/psychiatrists of any concerns or changes in mental status.
Responsible for delivery, observation, monitoring of compliance, and documentation of medication, as well as daily review of transfer of accountability report, risk management interventions and care planning.
Provide client-centered advocacy to facilitate access to/modification of existing resources so as to meet individual needs and assist clients in identifying/advocating for their civil and legal rights.
Encourage use of and facilitate access to wanted/needed services, supports, and resources, including those in the broader community.
Work collaboratively with ethno-racial communities and organizations to facilitate appropriateness of services for the members of these communities.
Assist clients to identify, build, and maintain a natural support system.
Responsible for transporting clients in the community and, on occasion, outside of the geographical area.
Assist clients with their activities of daily living (ADL) such as budgeting, shopping, meal planning/preparation, housekeeping, use of transportation, personal grooming, laundry, keeping appointments, etc.
Aid clients/participants in securing housing and support services. Including, conducting SPDAT’s (Service Prioritization and Decision Assistance Tool).
Provide support, monitoring and advocacy for clients to obtain and maintain housing.

Required Qualifications:

Minimum two (2) years combined education and experience in the human services field (three-five years preferred).
Registration in good standing as a Registered Social Worker (RSW) with the Ontario College of Social Workers and Social Service Workers.
Must have one of the following:

o Bachelor’s or Master’s degree in Social Work with equivalent skills and experience, or;

o Other relevant Bachelor’s or Master’s level training (e.g., Education, Counselling, Conflict Resolution etc.) with equivalent skills and experience.

Experience in mental health and/or addictions (community-based setting preferred).
Submission of a Vulnerable Sector Check (VSC), including a Criminal Record Check (CRC) in good standing
Must have access to a vehicle, a valid Ontario driver’s license, and adequate automobile insurance.
Fluency in French or another second language would be an asset.
Legally entitled to work in Canada.

Salary Scale:

RN: $60,005 - $65,957 *per annum

Hours of Work:

Thirty-five (35) hours per week.
Rotational shift work based on seven (7) day staffing schedule.
Program provides service 365 days per year.
Able to work within a flexible framework to respond to client needs.
