With a degree in a related health or social services field and at least three years of direct experience, you bring a deep understanding of child and youth mental health, developmental challenges, and the systems that support them.
You excel in building collaborative relationships—with families, caregivers, and community service providers—to develop and implement Coordinated Care Plans that are family-centered, goal-driven, and reflective of individual needs. You are skilled at facilitating service coordination meetings, navigating funding and referral processes, and advocating for appropriate resources to enhance quality of care and support.

The ROLE
As a coordinated service planner, you will be reporting to the manager for Coordinated Service planning. Work collaboratively with families and community providers to support and develop a coordinated Care Plan that reflects the goals and needs of families with complex and/or multiple special needs.
You will be responsible and accountable for ensuring the development of the plan and coordination of services and supports involved with families. You will collaborate with child/youth, families, and community partners to identify strengths and needs.
You will Gather information and in collaboration with caregivers and service providers determine appropriate level of support including funding, equipment, in-home safety or complex needs. Initiate internal and external referrals in consultation with the family. Facilitate and coordinate communication with service providers (family, schools, etc.) and clarify client needs.
You will Support development of a family centered collaborative coordinated service care plan with specific goals driven by the family. Facilitate service coordination meetings with family and support team during service in accordance to the need of the family and planning, while ensuring the necessary service-providers are present to encourage community collaboration and knowledge transfer where applicable and Provide timely Action Plans (Meeting Minutes) to family and attendees post- service coordination meetings.
You will prepare appropriate documentation and present it to the Connections Halton Committee as needed and prepare appropriate documentation for Complex Special Needs. Funding resources and follow community processes for requesting OPR, out of area residential placements, and/or the use of CARS and CPRI assessment requests.
You will collaborate closely with families and support teams to identify and connect with appropriate respite providers that align with the unique needs of the child/youth and their caregivers. You will regularly maintain and update respite tracking systems to ensure accuracy in billing and invoicing processes.
You will be responsible for scheduling and coordinating appointments across internal teams, community partners, and families, ensuring seamless communication and time management. Building and maintaining strong, supportive relationships with clients and their families will be key in fostering trust and collaboration.
You will provide specialized, dedicated, and at times urgent, resource coordination between multiple service providers, ensuring continuity and consistency in care. You will also cultivate strong working relationships with external agencies and service providers, monitoring follow-through on all planned actions.
Complete all documentation, reporting, and clinical records promptly and in accordance with established standards. Ensured the accuracy and integrity of case notes, statistics, and documentation relevant to the Coordinated Service Planner’s responsibilities.
You will prioritize client safety by adhering to established policies and guidelines that ensure consistency in the delivery of quality and safe care and, by identifying and reporting any client safety incidents. You are also committed to supporting client-centered care by adhering to ROCK’s principles.
Who we’re looking for:
A university degree in a related health care field or a Social Service/Child Youth Worker diploma.

- Proficient in the use of technology and working in a virtual environment; MAC OS operating systems; strong MS Word and Excel competencies are essential; expertise in the use of PowerPoint is an asset
- Ability to work independently and as a team
- Reliable transportation for traveling within the Halton Region in alignment with the program needs
- UMAB is an asset; additional training is required upon first 6 months of hire date.
- First Aid/CPR-C Certification; required upon first 3 months of hire date.
- Ability to delegate and guide collaboration within varieties of teams
- For all employees/agents of ROCK, a current (within 3 months) Criminal Reference Check (Vulnerable Sector Check, where working with the vulnerable population) and every 3 years.
- A valid Ontario License and access to use of a reliable vehicle.
- Bilingualism in French and English is an asset. The ability to speak any second language is valued.
