Smiths Falls and Brockville (Smiths Falls 80% - Brockville 20%)

The Case Worker plays a central role within the HART Hub, providing comprehensive, client-centered support to individuals accessing housing, recovery and related social services. This position involves assessing client needs, coordinating multidisciplinary care and ensuring continuity of support across community, clinical, and housing partners. Working within a collaborative and trauma-informed framework, the Case Worker helps clients navigate complex systems, access appropriate resources, and build the skills and stability needed for long-term recovery and well-being.

Responsibilities and Duties:

Establish a therapeutic relationship which is client-centered and based on a partnership.
Help individuals with severe and persistent mental illness and/or addiction to achieve their highest level of functionality possible in the least restrictive setting.
Provide interventions that coordinate client services in a fragmented addiction and mental health system.
Maintain a constant and ongoing support for clients even when their needs and use of services change.
Work with the client to develop a strategy/plan to reduce the client’s risk of experiencing a mental health crisis, including what actions by who and when, if client does experience a crisis/challenge in their recovery process.
Maintain accurate and up-to-date client files as per client management software so that the information is current for anyone needing to access information on the client.
Work with client to increase personal skills both internal and external to assist in symptom management and facilitate recovery process.

Qualifications:

Education and Training:

Graduation from a recognized post-secondary institution with a three (3) year diploma or degree in a relevant, health-related discipline of study; University preferred. A two (2) year diploma combined with appropriate and relevant experience may be considered.
Registration with a professional college is preferred.
Current First Aid and CPR Certification required.
Applied Suicide Intervention Skills Training is preferred.
Course/workshop in concurrent disorders or equivalent (SMART Recovery Facilitator Training) preferred.
Completion of formal training in Motivational Interviewing preferred.
Safe Management Systems training (Crisis Prevention Intervention, or Non-Violent Crisis Intervention training) required.
A valid Ontario Driver’s License, access to a vehicle, and vehicle insurance with a minimum of $2,000,000 liability.
Basic computer and keyboarding skills for data entry and client record notes.
Current satisfactory vulnerable sector criminal reference check.

Relevant Experience, Knowledge, and Skills:

Two years’ relevant experience providing services to adults with chronic mental health and/or substance use issues; preferably in a community setting.
Group facilitation experience is considered an asset.
Demonstrated crisis intervention skills/experience and ability to respond to crisis situations as they arise.
Experience in program development and evaluation an asset.
Experience using evidence-based interventions, including Motivational Interviewing, Cognitive Behavioral Therapy and Rational Emotive Behavioral Therapy.
Ability to relate to clients in a caring, respectful, and non-judgmental way.
Effective communication, interpersonal, and conflict resolution skills with the ability to establish and maintain positive relationships.
Ability to advocate for clients who are experiencing mental health and substance use health concerns.
A clear understanding of and ability to demonstrate ethical and professional standards, boundaries, and judgement.
Ability to work independently and as part of a multi-disciplinary team.

Pay: $33.84-$35.88 per hour

Expected hours: 37.5 per week
