Your Opportunity:

This is an exciting opportunity for a Social Worker I in the Adult Inpatient program at the Centennial Centre for Mental Health and Brain Injury (CCMHBI) in Ponoka. CCMHBI is a campus of specialists and health professionals who are committed to integrating multi-disciplinary care and treatment to ensure the best outcomes for patients and their families. This Social Worker I position requires a highly motivated individual who is keenly interested in working with people. Strong interpersonal, organizational, and communication skills are large contributors to success in this position. The Centennial Centre for Mental Health and Brain Injury is a state-of-the-art centre that treats and cares for people with psychiatric disorders and those who have suffered a brain injury. Located in the community of Ponoka, within the parkland region of central Alberta, the Centennial Centre is one of the most modern mental health referral centers in western Canada. Reporting to the Unit Manager, the Social Worker I will perform work of limited scope and complexity which generally involves gathering and documenting client information, providing basic client support, basic assessments of clients’ and families’ social and economic environments, and making client referrals to agencies. This position may assist Social Worker II and IIIs. The Social Worker will inform the treatment team of all interventions involving patient care and take directions from the treatment team. Description:

As a Social Worker I, you will be a member of a multi- disciplinary team that provides mental health services to individuals, families and/or groups at a community based facility. You will be responsible for the initial screening of patients by gathering and documenting client information, basic assessment and providing support to clients and their families' short-term needs. You will liaise with and refer clients to appropriate community and government services and support, along with participating in case conferences and team meetings.
