Full-Time Permanent 1.0 FTE (37.5 hours/week)
Reporting to the Program Manager or designate, the Registered Dietitian provides patient centred nutrition care as part of an interdisciplinary primary care team. Working to full scope of practice, the Registered Dietitian assesses, develops, implements, and evaluates evidence-based nutrition care plans, with a focus on chronic disease prevention and management. The RD also serves as a nutrition resource for patients, physicians, and the broader health care team to support high-quality health outcomes.
KEY RESPONSIBILITIES & DUTIES
- Provide evidence-based nutrition care focused on the prevention and management of chronic disease within a primary care setting.
- Conduct comprehensive nutrition assessments, integrating medical and nutrition history, anthropometric data, laboratory results, lifestyle factors, psychosocial considerations, social determinants of health, and readiness for change.
- Develop, implement, monitor, and evaluate individualized nutrition interventions and care plans using patient centred approaches and behaviour change strategies, including motivational interviewing.
- Provide one-to-one and group nutrition counselling and education to support conditions such as diabetes, cardiovascular disease, gastrointestinal disorders, weight management, and eating disorders.
- Build and maintain therapeutic relationships with patients, families, and caregivers while addressing barriers to health, including food insecurity, cultural considerations, and health literacy.
- Develop and deliver nutrition education programs, workshops, health promotion initiatives, and patient self-management programs.
- Collaborate with physicians and interdisciplinary team members, including nurses, mental health coordinators, social workers, exercise specialists, psychologists, pharmacists, and community partners, to coordinate patient centred care and facilitate referrals as appropriate.
- Maintain accurate, timely clinical documentation and provide consultation reports outlining assessment findings, recommendations, care plans, and patient education.
- Participate in team meetings, clinical best practice discussions, policy development, and quality improvement initiatives to support excellence in patient care and service delivery.
The Dietitian will work:
1.0 FTE – 37.5 hours per week
Workdays are typically 8am-4pm, or 8:30-4:30, Monday to Friday
The primary work location is the EOPCN Central office. However, there is a requirement for employees to have a private workspace at home to facilitate remote work, particularly for virtual appointments with clients.
QUALIFICATIONS:
- Minimum of 2 years of clinical Dietitian experience. Candidates that have worked in a primary care or community setting are preferred. Please note that all candidates for the above position must have:
- Bachelor of Science degree with a major in Foods and Nutrition
- Completion of an accredited dietetic internship program with a major in Foods and Nutrition
- Current registration with the College of Dietitians of Alberta
- Current CPR (Heart and Stroke Foundation BLS Level C- Health Care Provider)
- Practice with a weight-inclusive approach to nutrition care
- Experience with counselling pediatrics (or desire to learn) is considered an asset
- Experience with providing nutrition care via phone and video is considered an asset
- Experience facilitating group education sessions, including programs such as Craving Change, is considered an asset
- Experience developing patient education material, group classes, and health promotion programming is considered an asset.
- Demonstrated knowledge of computer software such as electronic medical records, Microsoft Office (including PowerPoint)
- Excellent interpersonal and team skills
- Clear communication, critical thinking, and organizational skills
- A Criminal Record/Police Information Check with Vulnerable Sector check
