POSITION SUMMARY:
JBH has an exciting full-time opportunity for an enthusiastic and knowledgeable Nurse Practitioner who wishes to work in the vibrant Burlington area providing care to Geriatric Patients with a focus on dementia/delirium.
The Nurse Practitioner Extended Class (NP-EC) is a Registered Nurse with additional educational preparation and experience who possesses and demonstrates the competencies to autonomously diagnose, order and interpret diagnostic tests, prescribe pharmaceuticals, and perform specific procedures within his/her legislated scope of practice as delineated by the College of Nurses of Ontario. NP’s have an essential role in advancing Joseph Brant Hospital’s Strategy to provide high quality and timely care to the patients and their families. The Nurse Practitioner utilizes a comprehensive theoretical knowledge base and an advanced level of clinical competence in caring for (list specialty population) patients in an inpatient setting in collaboration with Physicians and the interprofessional team. The NP will be expected to provide comprehensive health assessment and diagnosis, develop and implement a plan of care, provide health counseling and emotional support in the areas of illness and injury prevention, complications, and health promotion.
The NP-EC will provide leadership and promote excellence in patient-centered nursing care through expert practice, education, consultation and research.
Responsibilities
The NP role is derived from blending clinical diagnostic and therapeutic knowledge, skills and abilities within a nursing framework that emphasizes holism, health promotion and partnership with individuals and families, as well as communities.
Direct Comprehensive Care
- Obtains and documents a comprehensive health history, performs a physical examination on patients requiring assessment, collaborates closely with physicians and other members of the interprofessional team and coordinates care involving other healthcare providers such as the Home & Community.
- Uses advanced assessment and evaluation skills to develop/initiate/modify the health care plan, determine and implement required diagnostic testing and initiate/adjust/discontinue pharmacological and non-pharmacological therapies in accordance with scope of practice as defined by the CNO for NP practice, legislation, and JBH policy and procedures.
- Initiates referrals for consultation (medical and non-medical) in collaboration with physician as indicated.
- Provides consultation to nursing, medicine and other members of the patient care team.
- Demonstrates sound clinical reasoning and current evidence in decision-making, planning and implementing care.
- Participates and/or facilitates communication between patients and patient care team related to plan of care.
- Develops and implements innovative approaches for complex practice issues and facilitates optimal outcome management of patient care.
- Participates in the development, implementation and evaluation of best practices within program.
Education
- Contributes to the development, implementation and evaluation of patient education/education programs.
- Develops information and education for patient/family/significant others with the focus on health promotion, disease prevention and impact of illness.
- Conducts educational teaching sessions with patients and/or families with a focus on health promotion, disease prevention, and impact of illness experience on the patient and family.
- Identifies and addresses learning needs of patient and significant others.
- Advances the process of knowledge transfer for nursing staff by being a role model in the integration of best practices.
- Participates in regular evaluation of delivery and management of care.
- Enhances personal and professional development through reflective practice and by attending educational opportunities and /or conferences in clinical specialty, research and scholarship, and /or leadership topics.
- Collaborates with nurses to stimulate interest and provide innovative learning opportunities to integrate research findings, theory, and technique and that meet and respect needs and requests of patients/families/significant others.
- Provides mentorship for undergraduate and graduate nursing students, as well students from other health disciplines, in their clinical practicum experience.
- Develops information and education for patient/family/significant others with the focus on health promotion, disease prevention and impact of illness.
Quality Improvement/Research
- Developing and leading quality improvement projects related to patient's safety, evidence –base care and collaborative practice
- Conducts critical analysis of current research focusing on patient care as it relates to focus population
- Develops and/or utilizes appropriate assessment instruments to measure clinical outcomes as it relates to focus population
Professional Practice/Leadership
- Provides leadership in the development, implementation, monitoring and evaluation of quality initiatives.
- Participates and provides leadership in the strategic planning process for the delivery of medical care.
- Supports educational initiatives directed at nursing and allied health to promote and provide medical care that meets best practice standards.
- Act as a consultant and resource to nursing administration, (program)members of the health care team and external groups in the process of decision-making, strategic planning and problem solving
- Participates in the development/revision of practice guidelines, protocols, policies and procedures.
- Promotes the integration of the nursing viewpoint in program planning, policy-making and problem solving at the program and corporate levels of the organization.
- Develops and uses tools and processes to evaluate quality of nursing practice at the individual, program, and organizational level.
- Advocate for and contributes to the development, integration and evaluation of policy that impacts quality of patient care.
- Act as a change agent by advocating for and implementing new initiatives
- Act as a role model and champion to identify and integrate best practices into daily clinical care and activities to foster the delivery of safe and exemplary care
QUALIFICATIONS:
- Current registration in the Extended Class (NP) in good standing with the College of Nurses of Ontario required.
- A Masters Degree in Nursing and completion of an Acute Care or Primary Care Nurse Practitioner Program required.
- Member of the NPAO preferred.
- Member of the Registered Nurses Association of Ontario preferred.
- Current or recent clinical experience of at least three years working in acute care with a geriatric focus required.
- Currently holds a cross appointment at an Ontario University an asset.
- Research experience an asset.
- Program planning, development and implementation experience an asset.
- Experience in change management an asset.
- Demonstrated advanced clinical assessment, diagnostic and reasoning skills.
- Demonstrated knowledge and experience related to adult education.
- Demonstrated ability to function effectively in a collaborative multidisciplinary team.
- Demonstrated understanding of quality improvement processes and LEAN methodologies.
- Demonstrated effective interpersonal, communication and team building skills.
- Demonstrated excellent verbal and written presentation skills.
- Demonstrated reliable attendance, punctuality and availability.
