Lennon House Case Worker
Position Overview
The Lennon House Case Worker supports residents in developing individualized case plans that promote stability, recovery, accountability and successful community reintegration. This role provides one-to-one support, completes ongoing needs assessments, coordinates referrals, maintains accurate documentation and works collaboratively with the Operations Manager, Resident Manager, peer support staff and community partners.
Working within a holistic recovery-oriented environment, the Case Worker helps residents identify goals, strengthen recovery skills, access services and build the practical life capacities needed for sustained recovery.
Key Responsibilities
● Support resident admissions and complete initial intake support
● Meet with new residents within 24 hours to begin building rapport and developing a case plan
● Maintain regular one-to-one meetings to assess needs, monitor progress and support goal planning
● Develop and update case plans, relapse prevention plans, discharge plans and behavioral support plans
● Maintain accurate case notes, reports, resident files and required documentation
● Coordinate referrals for housing, counselling, medical care, employment, legal support and community services
● Ensure all consent forms, releases and file records are completed and maintained
● Collaborate with the Resident Manager regarding resident concerns, behavioral expectations and case planning
● Support medication coordination including pharmacy communication, refill tracking, medication counts and documentation
● Assist with medical appointments and healthcare coordination as required
● Respond to resident emergencies through support, intervention, documentation and escalation when necessary
● Participate in shift debriefs and communicate relevant resident updates to staff
● Support behavior modification plans and discharge planning when required
● Coordinate with Resident Manager on phases of stay, reapplication timelines and follow-up requirements
● Maintain communication logs and tracking systems
● Model professionalism, healthy boundaries and recovery-oriented behavior
● Complete other duties as assigned
Skills and Competencies
● Strong communication, conflict resolution, organizational and time management skills
● Compassionate, non-judgmental and professional approach
● Understanding of addiction recovery, behavioral health and trauma-informed practice
● Strong case management, assessment and documentation skills
● Ability to maintain confidentiality and professionalism with sensitive information
● Ability to remain calm and solution-focused during crisis situations
● Strong interpersonal skills and ability to work collaboratively within a team
● Ability to build rapport while maintaining accountability and structure
Qualifications
● Post-secondary education in a related field or equivalent combination of education and experience
● Minimum 2–3 years of experience providing case management or direct support services
● Knowledge of recovery-oriented environments and community resources
● Experience supporting individuals with addictions and co-occurring mental health challenges considered an asset
● Strong organizational, administrative and documentation skills
● Proficiency with Microsoft Office, Windows and Google Workspace
● Current Police Vulnerable Sector Check
● Valid driver’s license and clean driver’s abstract
Assets
● Knowledge of recovery-based or 12-step programming
● Experience with assessment, treatment planning, discharge planning and clinical documentation
● Experience in residential recovery or supportive housing environments
● Current First Aid/CPR, NVCI or Mental Health First Aid certification
