The Mahingan Sagahigan Health & Wellness Centre provides holistic, culturally grounded health and wellness services to the Wolf Lake First Nation community, guided by Anishinabe values of respect, balance, and community care.
We are seeking a compassionate, organized, and community-focused individual to join our team as a Community Health Representative (CHR). The CHR plays an important role in supporting the health and well-being of community members through outreach, education, advocacy, prevention initiatives, and culturally safe health programming. This role supports community members across the lifespan and works alongside individuals, families, Elders, and community partners to promote health, strengthen access to services, and encourage wellness in a culturally meaningful way.
Position Summary The Community Health Representative acts as a bridge between community members and health services by helping individuals access supports, promoting healthy lifestyles, and creating opportunities for education and wellness within the community. The successful candidate will build trusting relationships within the community and support initiatives that reflect the values, strengths, and needs of Wolf Lake First Nation members.
Key Responsibilities
Community Health Promotion & Education
- Deliver culturally safe health education and wellness information to individuals, families, youth, and Elders.
- Promote healthy lifestyles and support awareness initiatives related to nutrition, physical activity, chronic disease prevention, mental wellness, substance use prevention, harm reduction, and overall wellness.
- Assist with the development and distribution of educational materials and community health resources.
- Support culturally grounded activities and wellness initiatives that incorporate traditional teachings and community values.
- Participate in community events and health promotion activities.
Client Support & Community Outreach
- Assist community members in accessing healthcare services, programs, appointments, and community supports.
- Conduct outreach and follow-up with clients requiring additional support.
- Help identify barriers to care and advocate for community members when appropriate.
- Support referrals and connect clients with internal and external services.
- Build trusting and respectful relationships with Elders, youth, families, and community members.
Program Coordination & Support
- Assist in organizing and coordinating community wellness initiatives, clinics, workshops, screening programs, and health events.
- Support community health assessments, surveys, and program evaluations.
- Maintain accurate documentation, statistics, activity reports, and program records.
- Assist with monthly and annual reporting requirements as required by funding agreements.
- Monitor program activities and contribute ideas for ongoing program improvement.
Team & Community Collaboration
- Work collaboratively with nursing staff, mental wellness workers, community support staff, and external service providers.
- Participate in staff meetings, case discussions, and health planning initiatives.
- Support integrated approaches to care that address physical, emotional, mental, and spiritual wellness.
- Represent the Health & Wellness Centre professionally while maintaining confidentiality and cultural respect.
Qualifications
- Diploma in Community Health, Human Services, Social Services, Indigenous Studies, or a related field is considered an asset.
- Equivalent combinations of education, community knowledge, and relevant experience may be considered.
- Previous experience working in community health, outreach, health promotion, or Indigenous community programming is considered an asset.
- Knowledge of First Nation health issues and culturally safe approaches to care.
- Understanding of community-based wellness and prevention models.
- Proficiency with Microsoft Office and general computer skills.
- Valid driver's license and reliable transportation may be required.
- Clear CPIC and Vulnerable Sector Check required prior to employment.
