About This Role: This position provides support to the Sisters of the Holy Names of Jesus and Mary (SNJM) residing in the Windsor, Ontario region. Consults and advises regarding healthcare needs and services and performs all essential functions of care coordination. Under supervision of Director of Healthcare Services, coordinates daily support services as needed by individual Sisters and helps to enhance each Sister’s understanding of her own medical needs.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Assists and advises individual Sisters who have requested assistance or who have been identified as needing assistance with healthcare and related services to support their independence and individual preferences.
- Performs all essential functions of care coordination which include assessment, development of health and related needs-based plan, implementation, monitoring, evaluation, and adjustment.
- Transports Sisters to medical appointments when needed and provides advocacy in the current healthcare environment, including accompaniment requested by a Sister for important appointments, during health transitions, for medical testing, surgery, etc.
- Transports/arranges transport/or meets Sister at the Emergency Department. Assists in care coordination and discharge planning.
- Coordinates with Director of Healthcare Services regarding healthcare providers (physicians, discharge planners, home health agencies, hospice, etc.) as appropriate.
- Assists in transitioning Sisters as needed to other levels of care.
- Serves as a resource to Sisters and their chosen support person(s) especially during times of health transitions, serious medical conditions and/or surgeries.
- Provides direct nursing care services and daily support services with the approval of the Director of Healthcare Services.
- Keeps Director of Healthcare Services (and Power of Attorney for Personal Care as appropriate) involved in a particular Sister’s health status, updated on a regular basis. Sends weekly summary to supervisor.
- Works closely with the SNJM Assistant Director of Healthcare Services in areas relating to insurance, benefits, and special orders regarding equipment, medications, and medical supplies.
- Documents pertinent clinical information and updates in the SNJM database in a timely manner.
- Maintains positive, professional working relationships with staff at care facilities to ensure optimal care of Sisters.
- Provides and/or assists with education of Sisters about health and wellness topics.
DIRECTLY SUPERVISES: Healthcare Coordinator, Personal Support Worker
INTERRELATIONSHIPS: Works in close collaboration with Sisters, United States Regional Health & Well-Being team, SNJM leadership team, Medical Providers, staff at care facilities and others as necessary.
QUALIFICATIONS: To perform this job successfully individuals must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skills, and/or ability required. Education: Graduation from an accredited nursing program; BScN (BSN) preferred.
Experience: Registered Nurse (RN) with 8–10 years of experience working with the geriatric population across various care settings.
Previous case management experience preferred, including development, implementation, and oversight of individualized care plans.
Certification / Licensure: Current, unrestricted registration as a Registered Nurse with the College of Nurses of Ontario (CNO).

- Understanding of and ability to maintain appropriate professional boundaries.
- Demonstrated knowledge of and experience coordinating community resources in the Windsor/Essex, Ontario area.
- Knowledge and ability to supervise in-home care.
- Must be able to function independently and have flexibility, personal integrity, and the ability to develop relationships and work collaboratively with Sisters, staff, providers, and external contacts at facilities.
- Excellent organizational skills and ability to prioritize and execute tasks in a timely manner.
- Demonstrated written and verbal skills to ensure effective communication.
- Ability to respond to urgent and/or stressful situations appropriately and calmly.
- Proficient knowledge of Microsoft Office and other computer software programs including, but not limited to Word, Excel, Outlook.
- Valid drivers' license and personal vehicle; good driving record.
- Ability to travel frequently within the Windsor, Ontario region to fulfill job responsibilities.
HOURS WORKED: Part-time, 24-30 hours/week.
Occasional evening and/or weekend hours as needed. Rare overnight travel may be necessary for training/conferences.
PAY RATE: Will be paid Salary/Exempt, at stated hourly equivalent. Exact salary to be determined on time of hire based on schedule.
