Community Services Coordinator

Hours: 40 Hours/wk | Monday-Friday (8am-4pm) | Full-Time - $30.74/hr
Actively seeking: 1 candidate for existing position.

Location: 316 Spruce Street South, Timmins ON

Living Space is a low-barrier, non-profit emergency homelessness shelter offering 24/7 support for individuals experiencing or at risk of homelessness. Lived experiences, culture, and perspectives are essential to fostering a healthy shelter environment for both staff and homeless community members on Treaty 9. Indigenous candidates are encouraged to apply.

Position Summary:

The Community Services Coordinator provides comprehensive client assessment, care planning, resource navigation, and advocacy to support individuals accessing shelter services. Working within a low-barrier, trauma-informed, person-centered, and harm reduction framework, the Community Services Coordinator partners with clients to identify strengths, address barriers, and coordinate services that promote housing stability, improved health, and overall well-being.

The Community Services Coordinator reports to the Operations Manager and works collaboratively with internal staff (Charge Worker and Client Care Workers) and external service providers to ensure delivery of coordinated, timely/responsive, and appropriate client-centered support. This role focuses on system navigation, service coordination, and advocacy, helping client’s transition successfully through healthcare, housing, income, legal, and community support systems.

Guiding Principles:

The Community Services Coordinator is guided by the principles of Housing First, trauma-informed care, harm reduction, cultural safety, equity, and client self-determination. The role promotes coordinated care, reduces barriers to accessing services, and empowers clients to achieve sustainable housing, improved health, and greater independence through collaboration, advocacy, and effective service coordination.

Requirements:

· Ability to speak Cree, Ojibway or French will be considered a high-priority asset.

· A 2-year diploma or degree in a related field (i.e. Social Services, Community Services, Human Services, Psychology, Sociology, Indigenous Studies, or another related field.) Equivalent combinations of education and relevant experience may be considered. Candidates with Lived-experience are encouraged to apply.

· Experience providing case coordination, resource navigation, housing support, discharge planning, or community-based client services.

· Experience working with individuals experiencing homelessness, mental health concerns, substance use, trauma, or other complex social challenges.

· Demonstrated knowledge of community resources and social service systems.

· Vulnerable Sector Criminal Records Check is required.

· Living Space has a Vaccination Policy that requires employees to take part in vaccination programs recommended by Public Health Unit.

Preferred Qualifications:

· Mental Health First Aid.

· ASIST or Safe TALK.

· Nonviolent Crisis Intervention (CPI) or equivalent.

· CPR and First Aid.

Core Competencies:

· Cultural humility / trauma-informed approach.

· Strong assessment, care planning and coordination skills.

· Analytical thinking skills: Ability to use sound judgement when making decisions and solve/avoid problems as they arise through ethical anti-oppressive practices.

· Knowledge of Housing First, harm reduction, recovery-oriented care, and person-centered service delivery.

· Excellent advocacy, communication, negotiation, and relationship-building skills.

· Ability to maintain organization and accountability while coordinating multiple priorities and managing a diverse caseload with a focus on collaboration and teamwork.

· Strong professional communication, documentation, organizational, and time management skills.

· Ability to work independently and collaboratively within an interdisciplinary team.

· Ability to maintain confidentiality effectively with the ability to uphold professional and ethical boundaries.

· Ability to develop and maintain a dependable attendance pattern being on-time and present for the full duration of all shifts.

Key Responsibilities:

Assessment and Care Planning

· Complete comprehensive client assessments to identify strengths, needs, risks, and goals.

· Develop individualized care plans in collaboration with clients, reflecting their priorities, preferences, and identified needs.

· Monitor client progress and revise care plans as circumstances change.

· Identify barriers to housing, healthcare, income, and community participation, and develop strategies to address them.

· Support clients in making informed decisions and achieving self-identified goals.

Resource Navigation and Service Coordination

· Connect clients with housing programs, healthcare providers, mental health and addictions services, primary care, income supports, legal services, employment programs, Indigenous services, and other community resources.

· Coordinate referrals and facilitate warm handoffs to internal and external service providers.

· Assist clients with completing applications for housing, financial assistance, identification, and community programs.

· Coordinate appointments and support clients in navigating complex service systems.

· Promote continuity of care by maintaining communication with community partners and service providers.

Advocacy

· Advocate for equitable access to services, housing, healthcare, and community resources.

· Support clients in understanding their rights, responsibilities, and available options.

· Advocate on behalf of clients when barriers to accessing services are identified.

· Encourage and strengthen client self-advocacy while providing support as needed.

· Build collaborative relationships with hospitals, healthcare providers, community agencies, landlords, and government programs to improve client outcomes.

Housing and Community Integration

· Support clients in identifying and securing appropriate housing opportunities.

· Coordinate housing applications and referrals.

· Assist clients in developing skills and connections that support successful community living.

· Facilitate transitions from shelter to housing by coordinating community support and follow-up services.

· Promote long-term housing stability through ongoing service coordination.

Interdisciplinary Collaboration

· Participate in case conferences and multidisciplinary team meetings.

· Collaborate closely with Charge Workers, Client Care Workers, Environmental Services Workers, healthcare providers, outreach teams, and community agencies.

· Share relevant information to support coordinated service delivery while maintaining client confidentiality.

· Contribute to discharge and transition planning with hospitals and other service providers.

Documentation and Quality Practice

· Maintain accurate, timely, and objective documentation, including assessments, care plans, referrals, progress notes, and service outcomes.

· Ensure documentation meets organizational, legislative, and privacy requirements.

· Monitor service outcomes and identify opportunities for continuous quality improvement.

· Participate in program evaluation and quality initiatives.
