Position Summary:
The HF Connecting Health Nurse Practitioner-Led Clinic (the “Clinic”) provides culturally competent primary health care services with a focus on mental health through collaborative services, education, advocacy and research. Working closely with the Clinical Director, interdisciplinary health professionals and the administrative team, the Nurse Practitioner will be responsible for providing direct patient care.
Major Job Responsibilities:
I.
1. Provide comprehensive clinical health assessments, diagnosis, nursing care and services to clients (individuals, families and groups) to promote healthy lifestyle and to prevent and/or reduce the incidence of disease and disability at the clinic and in the clients’ homes.
2. Manage care for patients with both episodic and chronic illnesses and assist in addressing both medical and psychosocial needs.
3. Work collaboratively in an interdisciplinary health team to provide comprehensive primary care to patients while demonstrating a high degree of emotional quotient, empathy and clinical expertise.
4. Develop, implement and evaluate medical directives.
5. Provide regular reports and updates regarding client care practices, trends, challenges and proposed solutions.
6. Develop and implement individualized plans of care based on patient needs, independently or in consultation with a collaborating physician and other members of the clinical team.
II.
1. Participate in the development, implementation and evaluation of the clinic’s quality improvement plan.
2. Develop collaborative relationships with local Nurse Practitioners and the regional NPAO chapter to support patient care needs, networking, resource sharing, and compliance with the College of Nurses of Ontario Quality Assurance requirements.
3. Liaise directly or coordinate another team member to link with community service organizations, agencies and hospitals to support the development of a primary health care network and reciprocal referral relationships.
4. Participate in the development, implementation, monitoring and evaluation of treatment, education, counseling and health promotion programs and services for individuals, families, and the community.
5. Participate in clinical projects and studies as required.
6. Demonstrate flexibility and willingness to work evenings and weekends as required.
7. Performs other related duties as assigned.
8. Participates in staff development, training and meetings as appropriate.
Work Conditions:
1. Full-time, 37.5 hours per week.
2. Travel within the community may be required.
3. Able to work flexible work hours, including some evenings and weekends

Education and other Professional Requirement:
1. Current certificate as a Nurse Practitioner – RN (EC - Primary Care).
2. Bachelor of Science in Nursing required. Master of Nursing preferred.
3. Current registration in good standing with the College of Nurses of Ontario.
4. Current registration with the NPAO.
5. Current Basic Cardiac Life Support (BCLS).
6. A satisfactory result of Vulnerable Sector Police Record Check is a required.
Experience:
1. 3-5 years of nursing experience in a community, hospital or primary care setting. Home-based care experiences an advantage.
2. Experience with electronic medical record system (preferably Telus PS Suites).
3. Demonstrated ability to work in a team environment and a multi-cultural setting.
Skills and other Requirements:
1. Strong English written and verbal communication skills; proficiency in Mandarin/Cantonese is considered as a strong asset.
2. Proficient in the use of electronic medical record systems, Microsoft applications and drug database programs.
3. Mental health and acute care experience is considered an asset.
4. Valid Ontario G or G2 Driver’s License and access to a reliable vehicle are required.
5. Knowledge of provincial health policy.
