As a Nurse Practitioner, you will play a central role in delivering holistic, culturally safe primary care to individuals and families. Reporting to the Director of Health & Social Services, you will work within a collaborative, interdisciplinary team that supports the full spectrum of health, including physical, emotional, mental, and spiritual well-being.
This role is suited for someone who looks beyond symptoms to understand the full story of each patient. You build relationships with empathy, respect, and integrity, creating a space where patients feel heard and supported. You are committed to culturally safe care and approach your work with curiosity, openness, and a mindset of continuous learning.
In a dynamic, community-based environment, you will work closely with physicians and a diverse care team, including community health, mental health and addictions, home care, and nutrition services, contributing to coordinated, person- and family-centred care. Whether supporting chronic disease management, promoting wellness, or guiding preventative care, your work is grounded in trust and meaningful connection. Through this, you will help strengthen access to care, improve health outcomes, and contribute to the overall well-being of the Millbrook community.
Collaborative, Community-Based Care
- Deliver comprehensive primary care as part of an interdisciplinary team, ensuring coordinated and person- and family-centred care
- Collaborate with physicians and allied health professionals to assess, diagnose, treat, and monitor patient health
- Contribute to continuity of care by connecting patients to programs, services, and community resources
Clinical Excellence & Patient Management
- Conduct thorough initial and ongoing patient assessments using strong clinical and diagnostic reasoning
- Develop and implement care plans that support both immediate needs and long-term health outcomes
- Provide chronic disease management, preventative care, and health promotion support
- Support patients in building self-management skills and confidence in their health journey
Health Promotion & Advocacy
- Champion a holistic approach to health that integrates physical, emotional, mental, and spiritual well-being
- Promote early detection, illness prevention, and education grounded in behaviour change and self-management principles
- Advocate for patients and contribute to equitable, culturally safe care practices

Education & Credentials
- Master of Nursing (Advanced Nursing Practice) or equivalent Nurse Practitioner program
- Current registration as a Nurse Practitioner – Family/All Ages (or eligibility) with the College of Registered Nurses of Nova Scotia
- Current Basic Cardiac Life Support (BCLS) certification
Experience
- Proven experience working as a Nurse Practitioner, ideally within community or primary care settings
- Minimum of 3-5 years of nursing experience across a variety of clinical environments
- Experience working with patients across the lifespan (families, children, adults, and seniors)
- Experience using electronic medical records (EMR) systems
Knowledge & Approach
- Strong understanding of health promotion, population health, and determinants of health
- Demonstrated ability to provide holistic, patient-centred care grounded in cultural awareness and safety
- Excellent clinical judgment, critical thinking, and problem-solving skills
- Ability to communicate effectively with patients, families, and interdisciplinary teams

- Manual dexterity to operate computers and related equipment
- Demonstrated professionalism, reliability, and a strong work ethic in a community-based care environment
Pay Scale: $59.47 - $67.17/hour. Compensation is based on education, skills and experience in accordance with the Nova Scotia Nurses Union pay scale.
Contract: 13-month contract, September 2026 - October 2027
Location: On-site at Millbrook Health Centre, 812 Willow St, Truro, NS
Schedule: Full-time or Part-time Monday to Friday, compressed work week. Some evenings and weekend availability will be required from time to time.
Travel: Home visits within the community and surrounding area.
