Community Liaison Officer : Position Summary
We are seeking a highly motivated and relationship-driven Community Liaison Officer to represent our home health organization within the healthcare community. This role serves as a vital link between hospitals, healthcare professionals, patients, families, and community partners to ensure seamless transitions of care and promote awareness of our home health services.
The successful candidate will be responsible for developing and maintaining strong referral relationships, supporting hospital discharge planning processes, identifying community partnership opportunities, and educating healthcare professionals and community stakeholders about our services. This position requires exceptional communication, networking, and organizational skills, along with a passion for improving patient outcomes and supporting individuals in safely remaining at home.
Key Responsibilities: Relationship Development & Community Outreach
- Establish and maintain professional relationships with hospital staff, discharge planners, social workers, case managers, physicians, rehabilitation facilities, retirement homes, and community organizations.
- Serve as the primary point of contact for referral sources, community partners, patients, and families.
- Conduct regular in-person visits and meetings with healthcare professionals to strengthen partnerships and promote organizational services.
- Represent the organization at healthcare conferences, networking events, community programs, and industry meetings.
- Identify and develop new referral sources and partnership opportunities within the community.
Patient Transition & Care Coordination
- Collaborate with hospital teams and healthcare professionals to assess patient needs prior to discharge.
- Facilitate smooth transitions from hospital to home by coordinating appropriate home care services and support resources.
- Ensure patients have access to required personal support services, nursing care, equipment, and community resources before returning home.
- Act as a resource for patients and families by providing education regarding available home health services.
Marketing & Business Development
- Develop and implement outreach initiatives to increase community awareness and service utilization.
- Create and distribute marketing materials, presentations, and educational resources to referral partners.
- Assist in the development of social media content and community engagement campaigns.
- Monitor market trends and identify opportunities for growth and service expansion.
- Support organizational growth through relationship-based business development activities.
Administrative Responsibilities
- Maintain accurate records of referral activities, partnership meetings, and outreach initiatives.
- Prepare reports on referral trends, community engagement efforts, and business development outcomes.
- Coordinate community events, educational sessions, and promotional activities.
- Collaborate with internal clinical and administrative teams to ensure consistent messaging and service excellence.
Qualifications: Education
- Bachelor's degree in Healthcare Administration, Business Administration, Marketing, Communications, Social Work, Nursing, or a related field preferred.
- Additional healthcare-related education or training is considered an asset.
Experience
- Minimum 2 years of experience in healthcare marketing, community relations, business development, discharge planning, home care, healthcare administration, or a related field.
- Experience working with hospitals, retirement homes, long-term care facilities, rehabilitation centers, or community healthcare organizations is highly preferred.
- Demonstrated success in relationship building, networking, and referral development.

Pay: $20.25-$22.50 per hour
