The Lakeshore Community Nurse Practitioner - Led Clinic (NPLC) is a dynamic community-based interdisciplinary primary health care clinic providing comprehensive primary health care services to individuals and families residing in Lakeshore who are unattached to other primary care providers.
Working within the interdisciplinary team, the registered nurse (RN) will have the opportunity to work to full scope and provide high quality nursing to patients consistent with the vision and philosophy of the NPLC and with the standards of the College of Nurses of Ontario. The RN will participate with the interdisciplinary team in the provision of primary health care, health promotion and disease prevention.

Schedule
Monday - Friday
Must be able to work 10 am - 6 pm up to two days/week.
Roles and Responsibilities
· Working with NP to complete new patient intakes: assisting and reviewing intake forms, organizing charts, completing initial intake appointments in collaboration with NP; obtaining health history, identification of illness screening needs, immunizations and preventative care recommendations.
· Perform patient assessments, triage patients, and monitor, document, and report changes in patient health status.
· Administer medications, immunizations, and treatments as prescribed or as authorized under medical directives
· Manage clinical supplies, maintain infection control standards, and prepare equipment for safe and effective daily operation
· Chronic disease management: Conduct in office or home visits (as needed) assessments-utilize appropriate assessment tools, provide health education/counseling and review chronic disease medications in collaboration with NPs. This includes routine diabetes, HTN and COPD assessment.
· System navigation-Assist in completion of forms including but not limited to ODSP, CPP, disability tax, return to work. Assist in gathering of supportive documents for any applications. Assist in ensuring patients get to appropriate appointments if required including reminder calls, assisting with organizing transportation. Contact patients upon discharge from inpatient units to confirm appropriate services are in place, any gaps in care to aim at decreasing return to ED. Set up in office appt with PCP as needed.
· Establish and maintain community partnerships to expand outreach services, increase access to health and social services and foster linkages to care, mobilize resources, and address social determinants of health to meet the complex needs of clients
· Assist medical secretaries with triaging and scheduling patients as needed. Contacting patients for diagnostic test review as indicated by NPs
· Health promotion: tracking preventative care and contacting patients to up date, completing health promotion resources (slides, handouts)
· Social prescribing/link worker
· Collaborate with social work and dietician for implementing and conducting group programming.
· Organize influenza vaccination clinics, assist with vaccination education to the public
· The above duties are representative but are not to considered all-inclusive
Experience and Qualifications
· This is not a Francophone designated position, however, LCNPLC is working towards increasing Active Offer Francophone primary care services to the Lakeshore community. Please indicate if you can offer primary care services in French.
· Bachelor of Science in Nursing degree
· Registration in good standing with the College of Nurses of Ontario and proof of adequate professional liability protection as required by CNO
· 2 - 5 years experience working in community-based health care settings (experience working at an interprofessional primary care team is preferred)
· Demonstrated exemplary oral and written communication and interpersonal skills
· Demonstrated ability to work independently and in an interdisciplinary team environment
· Demonstrated ability in conflict resolution and crisis intervention
· Demonstrated educational/presentation/facilitation and networking skills
· Experience working with people experiencing mental health or substance use issues, and with harm reduction as an approach to substance use
· Experience with electronic medical records system (PSSuites preferred)
· Experience with chronic disease management including health promotion and prevention education
· Experience with program planning, delivery and evaluation, particularly in the area of chronic disease management
· Experience with Quality Improvement planning
· Demonstrated understanding of and ability to navigate the health care system on behalf of patients
· Demonstrated ability to advocate for patients to ensure health care system responsiveness
· Satisfactory criminal reference check/vulnerable sector check
· Current Basic Cardiac Life Support
· Ability to travel within the community with current Ontario’s driver’s license
· Ability to work flexible work hours including evenings (up to two evenings a week until 6 pm)
· Adherence to Occupational Health and Safety policies and procedures as per applicable legislation
· Adherence to all policies and procedures of the NPLC
