Job Overview
We are seeking a dedicated and compassionate Registered Practical Nurse (RPN) to join our healthcare team. The ideal candidate will provide high-quality patient care across various settings, including nursing homes, hospitals, assisted living facilities, and home care environments. This role requires a strong foundation in nursing practices, excellent communication skills, and the ability to work effectively within a multidisciplinary team. The Registered Practical Nurse will play a vital role in promoting patient well-being, managing medical treatments, and supporting patients' individual needs.

Duties

Deliver comprehensive patient care in diverse settings such as acute care, dementia care, pediatrics, senior care, hospice care, and home care.
Administer medications accurately and safely in accordance with physician orders and established protocols.
Conduct patient assessments, monitor vital signs, and document findings using electronic health records systems such as Epic and Cerner.
Assist with procedures including phlebotomy, pain management, and laboratory testing.
Collaborate with physicians, therapists, and other healthcare professionals to develop and implement individualized care plans.
Provide emotional support and education to patients and their families regarding health conditions and treatments.
Ensure compliance with healthcare regulations, safety standards, and infection control protocols.
Manage case documentation efficiently to support continuity of care across various healthcare environments.

Requirements

Current registration as a Registered Practical Nurse (RPN) with the appropriate licensing body.
Proven experience in nursing home settings, hospitals, assisted living facilities, or home care is highly desirable.
Knowledge of medical terminology, medication administration protocols, and clinical procedures such as phlebotomy.
Experience working with diverse patient populations including seniors, children, or hospice patients is preferred.
Familiarity with electronic health record systems like Epic or Cerner is advantageous.
Strong communication skills with the ability to work collaboratively within a multidisciplinary team.
Ability to handle multiple priorities efficiently while maintaining attention to detail.
