Position: Aurora-Newmarket Family Health Team (ANFHT) Occupational Therapist

Qualifications: Appropriate education and relevant experience

Reporting to: ANFHT Executive Director and Lead Physician

Governing Body: The College of Occupational Therapists of Ontario (COTO)

Job Summary:

The FHT Occupational Therapist will play an integral role in the success of the Aurora-Newmarket Family Health Team (ANFHT) whose primary function is to work collaboratively with the members of the ANFHT, to provide holistic primary health care and wellness programs to the patients rostered to the Team’s physicians. The Occupational Therapist will support patient care and the delivery of programs, focusing on in-office and in-home assessments and participation in the delivery of care plans to maximize patient functionality and minimize the impact of illness.

Education and Qualifications:

· B.Sc. (Occupational Therapy) and registration with the College of Occupational Therapists of Ontario (COTO).

· Additional training or experience in care of the elderly, pediatrics and/or chronic disease management is considered an asset.

· Demonstrated clinical skills in assessment, program planning, implementation and evaluation.

· Minimum 3 years recent experience in a community, primary care or hospital setting.

· Knowledge of community resources.

· Assistive Devices Program (ADP) Authorizer for mobility devices is preferred, but not mandatory

· Valid driver’s license and reliable transportation for home visits.

Roles & Responsibilities:

· Work within the scope of practice of Occupational Therapy, as outline by the College of Occupational Therapists of Ontario, and follow the college standards for obtaining consent, performing assessments, intervention planning and documentation.

· Maintain a current working knowledge of evidence based assessment and intervention approaches through ongoing education and professional development

· Accept and prioritize all patients referred by the ANFHT physicians and/or Nurse Practitioner, in accordance with the FHT’s established referral protocol.

· Complete appropriate assessments (both at the clinic and in-home) and develop goals and an intervention plan with the patient including referral to appropriate community agencies or partners.

· Monitor the intervention plan and modify as necessary in order to achieve the patient’s goals.

· Document all patient care activities in the patient’s electronic chart in a timely manner.

· Participate as a clinical team member in the development and delivery of targeted programs.

· Share results of the patient’s assessments and interventions with the referring physician and other team clinicians as requested within a specified timeframe and in accordance with established protocol.

· Enable patients to develop the skills to live independent, safe, healthy and productive lives through occupational therapy interventions.

· Develop, update and maintain education resources for patients and for providers.

· Provide individual and group education sessions for patients and providers.

· Maintain current working knowledge of the Aurora-Newmarket FHT policies and procedures, ensuring that all relevant documentation is current and accurate.

· Promote awareness of the ANFHT services and programs

· Uphold a positive job attitude; maintaining a safe and healthy work environment

· Demonstrate a willingness to assist others working in a team-centered environment

· Demonstrate flexibility in accomplishing tasks

· Use the most effective and efficient method to deliver activities and services while improving quality

Referrals/Collaboration:

· Evaluate and document outcomes of individual patient care in collaboration with the team.

· Provide communication andnetwrok with hospital and community staff and other interdisciplinary team members to ensure smooth transition for care

· Referrals to community services, as appropriate

· Community outreach and linkages

· Participation in Southlake Community OHT

Skills and Abilities:

· Expresses self clearly & concisely – both written & verbal

· Demonstrated ability to work with a team and work independently

· Listens & responds effectively, be open and non-judgmental

· Work collaboratively and support other team members

· Ability to deal with multiple requests, calls and prioritize work

· High degree of accuracy and attention to detail
