About the Lu'ma BCH team

Lu’ma BCH has launched new housing support services for residents living with acute, complex, and concurrent medical conditions in our transitional and modular housing project. We are building a core team of inter- disciplinary Indigenous and culturally aware practitioners to lead and deliver care together in a culturally safe, wholistic, integrated way.

About the RSW position

You have completed a Bachelor degree of Social Work and are registered with the BC College of Social Workers. You have a passion for working with the First Nations away from home and urban Aboriginal community, experience doing social work in healthcare, experience in trauma and crisis counselling, and love working in a multi-disciplinary team. You are generous with your wide range of skills, tools, experience, and gifts to contribute to shared care and health plans. You genuinely enjoy relationship building with individuals you assist and have excellent professional communication skills working in a circle of caring. You have experience working with people suffering from concurrent mental and physical health issues. You are compassionate and genuinely interested in strength-based approaches and solutions.

You are available to commit to working in-person 5 days per week 8:30am-5pm at our complex care transitional housing program located in the DTES area of Vancouver.

Duties and Responsibilities
Responsibilities include being an active listener, listening to the client’s goals, assisting with social determinants, assist in completing government forms, crisis counselling services, charting, maintaining records of completed work in EMR, attending interdisciplinary clinical/case conferences, connecting clients with community services, programs and resources, liasing with hospital & healthcare centres, and advocacy. Working as a professional and being a care team contributor.

1. Listen to the client’s healthcare experience, conduct social work needs assessments through conversation with client. Identify the client’s coping abilities and provide clients with self-regulation tools and skills in cases of complex social emotional issues.

2. Provide trauma and crisis counselling by exploring the emotional response to identified problems, personal concerns and treatment objectives to facilitate change in coping style, behaviour, attitude, feelings and adaptive responses to an adverse life event. Provide referrals for long term counselling as necessary.

3. Attend case conferences as a member of an interdisciplinary team for client reviews, admissions and discharges by providing input from a social work perspective on a variety of care issues related to the client to ensure proper and effective treatment of the client.

4. Provide consultative and direct services to clients, team members and community health professionals.

5. Understand professional guidelines of confidentiality, conflict of interest, and has developed ethical and moral values.

6. Maintain client records by charting encounters, client goals, assessments, interventions, modifications, appropriate client information, progress reports, and scanning in documents and files to the EMR. Chart SOAP notes in accordance with established standards, policies and procedures in compliance with regulatory requirements.

7. Maintain a basic record of workload statistics by recording information into approved formats, and collating data.

8. Participate in and/or leads meetings and case conferences.

9. Participate in program evaluation/quality improvement by identifying opportunities for change and formulating recommendations to better improve social work services.

10. Participate / witness respectfully in cultural protocols and ceremonies with clients and team.

11. Perform other related duties, as assigned.

Qualification Requirements
Current full registration with the British Columbia College of Social Workers to practice.

Bachelor degree in Social Work from a qualified education provider recognized professionally in BC. Training in Non-violent Communication, Trauma-informed Care, and De-escalating Conflict

Valid BC Driver’s License and access to a personal vehicle for business-related purposes.

Experience, Working Knowledge

Two (2) years’ recent experience working with First Nations community members with concurrent health issues, mental health and substance use issues.

Knowledge and interest in culturally integrating care with team members.

Knowledge of local (i.e. Downtown Eastside) and regional initiatives, services, programs and resources that may accessed for client care,

Working knowledge of other health disciplines and their role in a shared care team.

Working knowledge of legislation and government policies and procedures that relate to the client. Experience and practice in trauma and addictions counselling.

Abilities

Demonstrated ability to conduct social work needs assessments.

Demonstrated ability have respectful conversations and honour information that will contribute to understanding the clients’ situation.

Demonstrated ability to communicate effectively both verbally and in writing with clients, families, team members and community agency personnel.

Demonstrated ability to adjust to new or unexpected events.

Ability to develop trusting and respectful relationships with team members and people you serve together. Able to be curious, objective and compassionate, not judgemental.

Demonstrated ability to deal effectively with conflict situations.

Demonstrated ability to work collaboratively as a member of an interdisciplinary team. Ability to operate a laptop and use Microsoft Office and EMR.

Physical ability to perform the duties of the position.

Able to self regulate oneself mood and emotions in stressful situations and maintain professional composure and communication with others.

Ability to daily engage in self-wellness both at work and at home. Ability to create space for culturally safe care.

Permanent Full-Time

$ 88, 000 per annum.

Monday to Friday, 8:30 am to 5:00 pm. 8 hours shift with a .5 unpaid meal break.
