The coordinator will report directly to the Senior Manager. The successful candidate will have an active nursing license to practice in Alberta and be in good standing with their licensing body. It is preferred that the person have 2-3 years related experience working under a harm reduction model as well as 2-3 years’ experience in a leadership role. The Coordinator will be responsible for providing daily assistance as needed in a trauma informed, harm reduction model which will include medication administration, wound care, enhancing quality of life, maintaining hygiene, managing symptoms, and assisting in activities of daily living. He or she will perform administrative duties for the HST including reviewing team-charting, scheduling team members, and helping the manager prepare stats.
Description of Duties and Responsibilities
· Oversees the recording of patient’s medical history, ensures accurate case notes are taken by all team members,
updates the George Spady Society client files, and documents information on FYI database
· Ensures the highest standards are applied across the HST
· Helps the Manager compile stats, organize the schedule, and resolve minor conflicts among staff
· Assigns daily tasks to HCAs on sites
· Co-ordinates care with SH case workers and external health care providers
· Takes and record measurements of blood pressure, temperature, heart rate, vital signs etc.
· Assists ailing patients in daily necessary activities such as eating, bathing etc.
· Administers injections, prescribed medications, enemas etc., clean and dress wounds and assist with other basic medical care tasks
· Provides emotional and psychological support when needed
· Occasional transportation and warm hand off clients to medical, wellness and recovery appointments
· Interacts with HST clients in a non-judgmental way, using de-escalation and rapport building techniques
· Assists in the daily cleaning duties (beds, floors, etc.)
· Participates in shift change meetings
· Co-ordinates with interdisciplinary team such as physicians, AHS staff, case managers, home care workers, etc.
· Provides necessary support in crises and emergencies
· Participates in team efforts to maintain a healthy physical environment for our clients
· Attends staff meetings as required
· Any other tasks assigned by the Senior Manager
Position Qualifications
· Proven experience as RN, LPN or similar role
· Ability to undertake a variety of diverse care tasks and work in a multidisciplinary team
· Understanding of confidentiality obligations and nursing best practices
· Experience working with people with complex life circumstances associated with substance-related disorders, mental health issues, mobility issues and homelessness
· Basic understanding of harm reduction principles based on dignity, hope and healing as individuals move forward towards improved health and well-being
· An open, accepting and non-judgmental attitude with an ability to build rapport with clients and staff
· Good knowledge of MS Office and data entry
· Excellent communication and interpersonal skills
· Compassionate and able to handle stress
· Demonstrates understanding of the addictions field
· Proven leadership skills
· Valid Driver’s License
· Own vehicle with $2 million liability insurance transport clients as needed
· Current Vulnerable Sector Police Information Check
· First Aid/CPR/AED Training
Hours of work: 8-Hour Day Shift work. Permanent Full -time 7:45am-4:15pm Monday-Friday, no weekends.
