The main purpose of this position

The Nurse Practitioner (NP) will provide in-person and virtual access to primary care services to vulnerable populations in Halton, including those with complex health care needs, mental health and addictions concerns and unhoused. The position will work as a team member of the Interprofessional Primary Care Outreach Team. The NP is responsible for ensuring that nursing care provided to clients meets all legislative requirements and practice standards of the College of Nurses of Ontario (CNO).

The main purpose of the Enhanced Primary Care Pathways and Integrated Care (EPPIC) Team

The EPPiC Team is responsible for providing one-to-one community-based primary care and clinical support across the Halton region supporting people who are unattached to primary care, unhoused and who experience complex health care concerns including mental health and substance use concerns, including Hep C, HIV/AIDS etc. The team will work primarily in the community meeting people where they are at, including shelters, supportive housing, street level outreach and key community partner locations. This team will include Nurse Practitioners, Registered Nurses, Social Workers, Peer Support Workers and an Occupational Therapist.

Responsibilities

Client Focused Work

Delivers primary care services within the Nurse Practitioner (NP) scope of practice as outlined by the College of Nurses of Ontario (CNO) and Bill 179, including comprehensive health assessments, physical examination, therapeutic management, consultation, collaboration, and referrals for those who use substances.
Assesses, investigates, diagnoses, and treats patients, utilizing critical thinking and evidence-based medicine and nursing as a guide to practice.
Provides and evaluates holistic patient care based on advance nursing and medical management knowledge and skills.
Discusses diagnoses with the client, including discussing relevant clinical information, treatment and the expected outcomes and prognoses.
Work with clients and their circle of care to develop a plan of care (Unified Care Plan) utilizing assessment findings, diagnosis, and evidence-informed practice.
Establishes a plan to monitor the client’s response to medication therapy, including withdrawal management and continues to adjust a medication/treatment and support based on assessment of the client’s response.
Consults with physicians for clients who have unstable/complex health conditions.
Performs physical exams, orders tests, diagnoses and treats substance use disorders e.g., prescribing withdrawal medications specific to their substance(s) of choice, safe supply and or anti-craving medications.
Writes prescriptions and orders laboratory tests within the NP scope of practice
Provides supportive counselling/short-term crisis management
Conducts essential home visits to clients living within the Halton Region
Ensures that quality of client care consistently meets accepted College of Nurses standards, the Regulated Health Professions Act, and SH policies
Utilizes a health promotion approach to addressing social determinants of health
Maintains complete and accurate records sufficient for timely communication with other professionals

Organizational Focused Work

Establishes collaborative relationships with other health care providers and community-based services; supports the health needs of individuals with a history of addictions, mental health and other chronic illnesses and refer to specialists and other community agencies
Coordinates service provision with other health care partners in the development of community care plans and wrap-around services
Participates in case conferences with other providers to ensure client service coordination
Refers clients to specialists, hospitals, or community resources as appropriate; makes appropriate and timely internal referrals to other SH programs

Knowledge and skills necessary to be successful in this role

Master's Degree in Primary Health Nurse Care Practitioner program
Successful completion of the Prescribing Narcotics and Controlled Substances course an asset
Completion of a Suboxone on-line education program an asset
Current registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class
Membership in specialty related organizations e.g., NPAO, RNAO an asset
Non-violent crisis intervention, First Aid, CPR and ASIST certifications are assets
Current vulnerable sector check
Valid driver’s license and access to a reliable vehicle
Minimum of three years of community health care and/or primary care experience required
Experience in addiction medicine and harm reduction
Excellent clinical skills
Thorough current nursing knowledge and proficiency in conducting a comprehensive health history, physical assessment, and construction of a nursing management plan
Project planning and evaluation skills would be an asset
Demonstrated ability, commitment to and knowledge of community addictions and mental health care
Extensive knowledge and understanding of harm reduction principles and practices, substance use and overdose response
Knowledge of evidence-informed guidelines, legislation, regulation, and organizational policies when prescribing controlled drugs/ substances (CDS)
Understanding of best practices related to the use of CDS and of other pharmacological and non-pharmacological therapeutic options
Ability to deal effectively with sensitive issues with clients and families using a harm-reduction and trauma-informed approach
Knowledge of health care facilities and community health services, social agencies etc.
Knowledge and understanding of clinical theories and practices, pharmacology, issues affecting the Halton community, including services and resources available for clients
Demonstrated understanding of the neurobiology of addiction
Advanced practice assessments and interventions for a diverse range of clients
Advanced health assessment skills to identify clients’ emotional, cognitive and behavioural states, as well as level of anxiety, crisis states, indices of aggression, self-harm, suicide, risk to others, competency to care for self, and signs of substance abuse, addiction, and withdrawal
Able to develop individual and team-based approaches and capacity within a multidisciplinary and complex support environment
Able to implement agency policy and procedures with reporting staff to ensure and maintain transparency, accountability, and compliance.
Proven ability to create meaningful partnerships, strong interpersonal skills.
Strong understanding of the Housing First philosophy and extensive knowledge of the unhoused population
Trauma-Informed training and experience
Confidence and experience in crisis intervention, mediation and de-escalation including non-violent crisis/suicide intervention skills.
Risk assessment skills and relevant experience is an asset.
Demonstrated complex problem solving in crisis-based work.
Able to mediate and negotiate situations.
Experience with the LOCUS / OCAN / VI-SPDAT assessments is an asset.
Must possess a broad knowledge of community resources and the ability to strongly advocate for clients and make professional referrals.
Understanding of intersectionality principles and practice, and how it may impact an individual’s experience with systemic oppression and support systems.
Excellent verbal communication and comfort with one-on-one dialogue
Excellent written and documentation skills.
Working knowledge of PHIPA is an asset.
Able to work independently and co-operatively in a busy multidisciplinary environment.
Self-motivated, comfortable working with minimal oversight
Proven ability to work on an Electronic Healthcare Record
Proven computer proficiency to facilitate the use of automated assessment tools and Microsoft products.

Working conditions

The EPPIC Team will be available in the community 7 days a week. The Team will work together to determine 7-day coverage. Each member will work 37.5 hours per week.
Position requires a mix of office environment and community outreach.

Starting salary $120, 000.
