ANNUAL SALARY : $55,002.39

POSITION SUMMARY:
Case Manager provides direct service and support to clients in the programs of Madison. Reporting to Program Manager – 81 Wilson Park, and the Mental Health Case Manager is responsible for providing recovery-oriented supports to clients in the following areas: needs identification, individualized assessment and planning, direct service provision, coordination and support, referral, monitoring, evaluation, and advocacy.
This role contributes directly to Madison's mission by addressing clients' social determinants of health and reducing barriers to housing, wellness, and recovery, while adhering to key performance indicators (KPIs) such as housing stability, client satisfaction, service accessibility, and goal attainment.
DUTIES, RESPONSIBILITIES, AND RELATED TASKS
- Assist in the planning, implementation and evaluation of the supports and services.
- Comply with the agency policies, standards, guidelines and procedures.
- Participate effectively and respectfully in staff and other designated internal meetings
- Work in cooperation with management and other staff members in the provision of efficient and coordinated services.
- Work from a recovery framework in achieving the organization’s mission, which is:to promote the health, independence, recovery and community integration of persons with mental health and/or addictions challenges through advocacy, education and the provision of a broad range of community based and housing support services.
- Complete assessments that identify clients’ needs, priorities and abilities through information obtained from the client, his/her formal and informal supports and the referral source utilizing agency and funder tools.
- Develop with each client a comprehensive individualized plan, which incorporates his/her goals and values and identifies all skills, resources and service requirements.
- Assessing the needs of clients/tenants and working with them and their supports to develop and implement meaningful housing support plans.
- Provide direct service including: ADL skills coaching, assistance in obtaining and maintaining housing, support, encouragement, counselling and feedback to enable clients to reach their goals, counselling and information to reduce the incidence, duration, intensity and impact of problematic symptoms, family support and education as appropriate to support the interests of the client.
- Maintain confidentiality and comply with agency standards and all applicable legislation.
- Provide information and strategies for health and mental health promotion and crisis prevention.
- Assist clients in identifying and advocating for their civil, legal, housing and other rights.
- Facilitate access to desired and needed services and resources, including community supports, social recreational supports, crisis intervention and treatment, among others.
- Assist clients to identify, build and maintain a social/informal support system, where desired.
- Collaborate with community service providers.
- Participate in and contribute to internal agency committees.
- Perform duties in a safe manner, in line with Madison’s Health and Safety policies and all applicable statutes.
- May assist in the training and supervision of students and/or volunteers.
- Monitor, regularly review and, if appropriate, revise client goals based on client’s individual needs and direction.
- Evaluate achievement of goals and client satisfaction using a variety of methods.
- Maintain client files and documentation as per agency policies, procedures, standards, guidelines and legal framework.
- Collection of data including: client demographics, service statistics, goal achievement and other systems that may be developed.
- Maintain relationships with networks, related professional organizations and other related service providers.
- Represent the mission, values and philosophy of Madison in the community including particular networks or meetings as requested.
- With the support and direction of Program Manager, develop an annual learning plan to move toward identified performance and professional development goals.
- Participate in regular supervision with Program Manager
- Attend relevant conferences, workshops and other in-service events as approved and/or directed.
- Become fluent in Madison’s policies, procedures, standards and guidelines of operation.
- Perform other related duties as assigned by Program Manager Supportive Housing Program or designate in the absence of the Manager.
KNOWLEDGE AND SKILLS
- Good understanding of relevant provincial, and local legislation and government policies, procedures and practices pertaining to services provided by the agency.
- Knowledge of the Residential Tenancies Act and the Mental Health Act.
- Competency in the provision of services offered by Madison.
- Competency in recovery-oriented and anti-oppressive practice.
- Proficiency in current approaches of harm reduction and client-centered principles.
- Knowledge of community resources connected to the provision of mental health and related services.
- Demonstrates diplomacy and negotiation skills in client and system advocacy.
- Demonstrated interpersonal and networking skills.
- Knowledge of skill coaching approaches.
- Demonstrated assessment and planning skills.
- Anticipates client difficulties or crises and applies crisis theory during these situations as appropriate. Use of appropriate skills and resources to intervene with aggressive behavior and suicidal behavior to maintain safety for all individuals.
- Knowledge of mental illness, medications and ability to assist clients with symptom management and medication advocacy as required.
- Knowledge of Accreditation Canada standards, quality improvement cycles, and client outcome measures is an asset.
QUALIFICATIONS
- University degree relevant to mental health & addictions case management is preferred.
- Must have a minimum of relevant diploma or certificate coupled with 3 years of experience in related adult mental health and case management.
- Group program development and facilitation is an asset.
- Understanding of homelessness/poverty and how these impact on people living with mental illness
- Able to work effectively independently, as well as part of a team.
- Specific knowledge of services and resources available in downtown Toronto an asset.
- A strong commitment to, and knowledge of, recovery principles and anti-oppressive practice.
- Knowledge of and proficiency in current approaches for adult mental health and addictions, harm reduction and client centered principles.
- Demonstrated knowledge and understanding of the Mental Health System to enable positive outcomes for the client and family.
- Demonstrated understanding and sensitivity to social and cultural diversity.
- Required to provide a satisfactory vulnerable sector background check.
- Training in CPR, First Aid and ASIST an asset.
- Computer literacy.
KEY PERFORMANCE INDICATORS (KPIs)
This position contributes to Madison’s organizational scorecard through:
- Client Goal Achievement – % of clients meeting individualized service plan goals
- Housing Stability – # of days housed; tenancy retention rate
- Client Satisfaction – Quarterly survey scores and feedback implementation
- Service Accessibility – Waitlist times and timely intake measures
- Health Outcomes – Reduction in ER visits, hospitalization rates, and crisis calls
- Compliance – Adherence to documentation, safety, and privacy standards as per accreditation guidelines
WORKPLACE ENVIRONMENT
81 Wilson Park program operates in a high-support housing environment. Staff should expect exposure to second hand smoke, substance use, and varied client behaviors related to mental health and/or developmental challenges.
HOURS OF WORK
- Monday to Friday (subject to operational needs)
o 8.00 am to 4.00 pm
o 9.00 am to 5.00 pm.
- Ability to work in high-demand, crisis-responsive environments is essential
- Travel between Madison program sites and community locations may occasionally be required
Staff may be required to support emergency responses and critical incidents outside regular duties Madison Community Services reserves the right to modify job duties and responsibilities based on operational requirements.
