Salary: $108,237.00 – $125,477.00 per annum depending on experience
Hours of work: 40 hours per week, Monday to Friday, within the hours of 8am to 5pm, with flexibility to offer some late or early morning visits to meet patient preference.

The Georgian Bay Family Health Team is seeking two skilled and compassionate Nurse Practitioners to join our team. The successful applicant will work closely with physicians in their primary care offices, located in Creemore and Collingwood. You will facilitate the continuity of primary health care by working collaboratively with our physicians, and our Interdisciplinary Health Professionals.

Georgian Bay Family Health Team (GBFHT) is a progressive, interdisciplinary team and an active partner in the South Georgian Bay Ontario Health Team (OHT), with strong community relationships across the region. Services include Lung Health, Diabetes Education, Heart Function Clinic, Healthy Aging, Nutrition Counselling, Cardiovascular Rehabilitation, Mental Health Counselling, and Telemedicine.
The first position will work closely with physicians in their primary care offices, located within the South Georgian Bay (Stayner, Creemore, Clarksburg, Wasaga Beach and Collingwood). You will facilitate the continuity of primary health care by working collaboratively with our physicians, and our Interdisciplinary Health Professionals.
The second position will be located at the Shoreline Health Clinic in Collingwood or Wasaga Beach. The clinic provides interim primary health care and supported attachment services for patients waiting to be rostered to local physicians. You will work collaboratively with our multi-disciplinary team, local physicians, and Interdisciplinary Health Professionals including, but not limited to, the Georgian Bay Family Health Team, South Georgian Bay Community Health Centre, Georgian Bay Family Health Organization.
Key responsibilities:
- Provide comprehensive primary care, including assessment, diagnosis, health screening, treatment, and follow-up
- Order and interpret diagnostic tests; prescribe medications as per regulatory standards
- Manage acute and chronic conditions using evidence-based practices
- Deliver preventive care, health education, and counseling to patients and families
- Collaborate with physicians and interdisciplinary team members to coordinate care
- Participate in team meetings, case conferences, and quality improvement initiatives
- Maintain accurate and timely documentation within the electronic medical record (EMR)
- Contribute to program development and community outreach as appropriate
Qualifications:
- RN in the Extended Class, current registration in good standing with the College of Nurses of Ontario
- Certification in Primary Health Care (PHCNP) essential
- Valid Ontario driver’s license and operational vehicle.
Experience:
- Primary care or community health setting, minimum 3 years experience preferred
- Telus PS Suite an asset
