Carefirst Seniors & Community Services Association is a non-profit charitable community services agency established since 1976. Our spectrum of services has grown from the delivery of Chinese meals-on-wheels service to a full range of community support services, covering the whole Greater Toronto Area.
With a staff of more than 550 members (including full-time and part-time staff) and the help of a pool of 850 active volunteers, we serve over 15,000 clients a year, including seniors, adults, youth with physical, social, emotional and health challenges. 3,000 of whom are “home-bound” and frail.
We are currently seeking an Occupational Therapist to fill a vacant position at our Geriatric Assessment & Intervention Network (GAIN) Team.
Terms: Full Time Contract (From August 4, 2026 to September 10, 2027)
Location: 300 Silver Star Blvd., Scarborough and community locations in the GTA
Pay range: $75,000 - $85,000
Responsibilities

Receive, review, triage, and process referrals from community, Ontario Health atHome, hospitals, etc., and liaise with community primary care providers to improve patient outcomes

Conduct Comprehensive Geriatric Assessment and cognitive assessments to assess patient function in activities of daily living, safety, cognition, responsive behaviours, physical function, and the need for assistive devices

Prepare service/care plans for individual clients, and participate in case conferences/weekly rounds to monitor and review client care plans, treatments and program evaluation activities

Match clients with appropriate care/support services, and provide educational and community resources to clients and their families

Maintain systematic and comprehensive records on client cases, group work/activities, financial reports, and evaluation reports

Prepare timely and accurate statistical, narrative and financial data reports to community and health care networks (i.e., Ontario Health atHome, GAIN Regional Office, Seniors Care Network)

Perform administrative and clinic management tasks such as, developing clinical policies and protocols; act as a trainer/mentor to provide in-service orientation, education and skills training to other team members and students; participate in Agency committees (i.e., Infection Prevention & Control Committee, Ethics Committee)
Requirements

Current registration in good standing with the College of Occupational Therapists of Ontario

Additional credential and training with respect to dementia care is an asset

Minimum 2 years of recent clinical experience in a health or community care setting, experience in geriatrics is an asset

Experience in developing, implementing, monitoring, and evaluating health promotion programs and activities preferred

Superior time management and organization skills

Strong clinical judgment and assessment skills to identify and prioritize complex problems and provide suggestions to improve the patient’s quality of life

Knowledge of community resources for the provision of health promotion and disease prevention related services and chronic disease management

Strong written and verbal communication, telephone, and interpersonal skills in a client service capacity

Sensitivity and awareness of cultural, racial, economic, and socially diverse communities of various ages

Proficient computer skills in Windows and Microsoft Office

Proficiency in English and a second language reflecting the community served, such as Cantonese or Mandarin is an asset

Valid First Aid & CPR Certification

Valid driver’s license and reliable vehicle for work

A valid vulnerable sector police record check completed within one year
