Full job description

Location: Fort McMurray, Alberta

Term: Permanent, Full-time

Job Summary:

Under the direction of the Post Majority Support Services Supervisor, the Post Majority Support Worker will be responsible for empowering our young community members who are aging out of care and/or are young adults formerly in care. The Post Majority Support Services Worker will develop and implement effective strategies to support the youth and young adults to reach self sufficiency within their communities.

Duties and Responsibilities:

Develop support programs that would aid the out of care young adult to easily access culturally appropriate supports and services that meet their needs.
Coordinate, schedule, and attend case conferences and appointments with community providers.
Collect and document data to track our members successes.
Assist in goal setting based on individual needs assessments.
Maintain regular contact with a network of service providers.
Participate in weekly staff meetings with the management team to evaluate the program and the community members’ progress.
Complete appropriate paperwork for program reporting and communication.
Coordinate family reconnection of out of care young adults.
Adhere to all ATC’s policies and procedures.
Maintain confidentiality and professionalism.
Other duties as assigned.

Qualifications and Skills:

Completion of a Degree or Diploma in Human Services, Social Work, or related field.
2 years experience in the human service or related field.
Experience working with individuals with complex needs.
Understanding of community resources within the communities ATC serves.
Knowledge of local Indigenous cultures and ceremonies will be an asset.
Professional/mature demeanor under stressful situations.
Demonstrates strong oral and written communication abilities.
Exceptional organizational skills with task prioritization, multi-tasking, use of timelines and time management techniques.
Must be able to work in a confidential and fast-paced environment.
Ability to speak Cree or Dene is an asset.

Requirements:

Clear Criminal Record Check with Vulnerable Sector.
Clear Child and Family Services Intervention Check.
Class 5 Drivers License and Drivers Abstract.
