Position Summary
Weight and metabolic health are among the most complex, most consequential, and most underserved areas of modern primary care. They sit at the intersection of biology, behaviour, environment, and medicine and they deserve a clinical approach that is as sophisticated as the science and as human as the person sitting across from you.
Stem Health is building a medical weight management and metabolic health program that reflects that complexity. We are looking for a physician with formal training or significant clinical experience in obesity medicine to lead this program delivering evidence-based, individualized, and compassionate care to members who are navigating weight, body composition, cardiometabolic risk, and metabolic health as part of their broader health journey.
In this role, you will conduct comprehensive metabolic assessments, develop individualized treatment plans incorporating lifestyle medicine, pharmacotherapy, and advanced diagnostics, and work closely with Stem Health's clinical team, care coordinators, and primary care physicians to deliver a seamlessly integrated metabolic health experience. This is not a volume program. It is a medical program and it will be practiced with the time, the data, and the team that medical weight management actually requires.
Key Responsibilities
Comprehensive metabolic assessment

Conduct thorough medical assessments for members presenting with overweight, obesity, or cardiometabolic risk including detailed weight history, eating behaviour patterns, physical activity history, sleep quality, endocrine evaluation, and psychosocial context.

Order and interpret a comprehensive metabolic workup including fasting glucose, HbA1c, insulin resistance markers (HOMA-IR), lipid fractionation, thyroid function, sex hormones, cortisol, adipokines, and inflammatory markers to establish a complete cardiometabolic risk profile.

Integrate body composition data from DEXA scanning (lean mass, fat mass, visceral adipose tissue, regional distribution) and resting metabolic rate (RMR) measured via indirect calorimetry into each member's clinical assessment grounding treatment decisions in objective metabolic physiology rather than weight alone.

Evaluate and document obesity-related comorbidities and contributing conditions, including insulin resistance, type 2 diabetes, metabolic syndrome, non-alcoholic fatty liver disease (NAFLD/MASLD), obstructive sleep apnea, polycystic ovarian syndrome (PCOS), hypothyroidism, and mood disorders affecting appetite and energy regulation.

Assess cardiovascular risk using validated tools (Framingham, SCORE2, ASCVD) alongside clinical findings, body composition, and metabolic data, contributing to the member's comprehensive cardiometabolic risk profile.
Individualized treatment planning

Develop personalized, evidence-based weight management plans that integrate lifestyle medicine, pharmacotherapy, behavioural strategies, and medical monitoring individualized to each member's metabolic profile, health goals, comorbidities, and preferences.

Prescribe and manage anti-obesity pharmacotherapy in accordance with current Canadian and international evidence-based guidelines including GLP-1 receptor agonists (semaglutide, liraglutide), GLP-1/GIP dual agonists (tirzepatide), bupropion/naltrexone, orlistat, and emerging agents with careful titration, monitoring, and ongoing clinical review.

Apply lifestyle medicine principles across nutrition, physical activity, sleep, and stress management developing practical, achievable, and medically grounded behavioural prescriptions tailored to each member's capacity and context.

Develop individualized caloric and macronutrient targets in collaboration with the clinical team, using RMR data and body composition findings as the evidence base for energy balance prescriptions.
Longitudinal monitoring and program management

Provide structured, longitudinal follow-up for all weight management program members tracking weight, body composition (DEXA), metabolic markers, pharmacotherapy response, side effects, and behavioural progress at defined intervals.

Adjust treatment plans based on objective progress data, metabolic response, member-reported experience, and evolving clinical evidence applying a responsive, individualized approach throughout the program rather than a fixed protocol.

Monitor for and manage pharmacotherapy side effects, contraindications, and drug interactions with precision and clinical care.

Recognize and address psychological contributors to weight and eating behaviours including emotional eating, binge eating disorder, and disordered eating patterns making appropriate referrals to the mental health team when indicated.

Track longitudinal cardiometabolic outcomes across the member's program, documenting improvements in metabolic markers, cardiovascular risk, body composition, and quality of life as part of the Unified Health Story.
Interdisciplinary collaboration

Work closely with Stem Health's primary care physicians to ensure weight management and metabolic health care is integrated into each member's broader primary care plan not siloed as a separate program.

Collaborate with the Exercise Physiologist on exercise prescription grounded in each member's RMR, body composition, cardiovascular fitness (VO2 max), and metabolic response designing progressive activity plans that complement pharmacotherapy and dietary intervention.

Partner with the Care Coordinator to manage program scheduling, follow-up cadence, laboratory requisitions, DEXA referrals, and communication with members between appointments.

Liaise with specialist colleagues (endocrinology, psychiatry, gastroenterology, sleep medicine, cardiology) for complex co-managed cases, ensuring seamless and coordinated care.

Contribute to Stem Health's clinical knowledge base by sharing emerging evidence in obesity medicine, metabolic health, and pharmacotherapy with the broader physician and allied health team.
Qualifications & Experience

Medical degree (MD or equivalent) and full licensure in good standing with the College of Physicians and Surgeons of Ontario (CPSO) required.

Residency training in Family Medicine (CCFP), Internal Medicine (FRCPC), or Endocrinology; additional fellowship or focused training in obesity medicine or metabolic health is a significant asset.

Minimum 3–5 years of clinical experience with demonstrated expertise in medical weight management, obesity medicine, or cardiometabolic risk reduction.

Strong working knowledge of anti-obesity pharmacotherapy including GLP-1 receptor agonists, GLP-1/GIP dual agonists, and adjunct agents with experience in clinical prescribing, titration, and management.

Familiarity with indirect calorimetry (RMR testing) and body composition assessment (DEXA) as clinical tools in metabolic health management.

Experience in lifestyle medicine, behavioural change counselling, and motivational interviewing as applied to weight and metabolic health.

Understanding of the psychological and behavioural dimensions of obesity, including eating disorders, emotional eating, and the weight stigma that many members will have experienced in prior healthcare encounters.

Experience working within a multidisciplinary team and integrating clinical findings across providers EHR proficiency required.

Exceptional communication and interpersonal skills are non-judgmental, warm, and able to build the kind of therapeutic relationship that supports sustained engagement with a medically complex and emotionally significant health journey.

Current CMPA professional liability coverage required.
