Reporting to the Program Lead/Manager within Clinical Services Integration and Patient Flow Supports, the Consultant is a provincial role accountable for supporting the implementation, adoption, and continuous improvement of initiatives that enhance access to referral and clinical pathway information across Alberta. This role will contribute to implementation activities associated with the Alberta Pathways Hub Optimization Project and support priorities of the Specialty Access Program Office. This position will work as part of a passionate and diverse team of system transformers that work towards strengthening integrated primary health care within Alberta’s provincial health system. Through partnership and co-design, the Consultant supports the implementation and adaptation of new and improved approaches to accessing referral and clinical pathway information that serve the needs of providers, patients, and communities. The role contributes to implementation efforts by establishing and leveraging collaborative relationships and shared working structures with system partners across organizations.
The Consultant supports alignment and coordination of Primary Care Alberta (PCA) priorities by bringing expertise in facilitation, partner engagement, human-centred design, patient engagement, change management, and quality improvement methodologies. The role works collaboratively with the Provincial Pathways Unit, pathway developers, implementation partners, providers, and system partners to support practice change and successful adoption of provincial initiatives. Using data, feedback, and implementation insights, the Consultant helps inform planning, evaluation, and continuous improvement activities. The successful candidate will apply practical experience in healthcare implementation and practice change to support clinics and providers in adopting new processes, tools, and resources. This includes supporting implementation planning, identifying barriers and enablers to adoption, facilitating engagement activities, and contributing to communication and training initiatives that advance system integration and improved access to specialty care services. The Consultant may also support and inform the strategic direction set out by leadership in Patient Navigation & System Integration (PNSI); and works to ensure PCA values, goals and principles are implemented and integrated within all projects and initiatives.

Required Qualifications:
Baccalaureate level degree with minimum of 3 years experience in a healthcare related field A minimum of 5 years of relevant experience with progressive responsibility in area of expertise in a related environment, within health care and preferably primary care Certification in quality improvement approaches such as AHS AIW Green belt, Lean Six Sigma, IHI, PROSCI preferred. Experience in planning and implementation pf projects/initiatives in a healthcare organization, preferrable in a Primary Care setting Demonstrated pattern of professional and personal development Proven ability to work collaboratively in a multidisciplinary team environment and with a broad spectrum of partner (both within and outside of the organization). Demonstrated ability to foster relationships and partnerships with internal and external partners, including patients/clients, families and caregivers. Demonstrated commitment to and application of patient engagement and Person-Centred Care principles and practices in a health care setting. Education and demonstrated abilities in change management, quality improvement practices and methodologies, and person-centred practice
Additional Required Qualifications:
Knowledge and demonstrated experience in facilitation or coordination of project management: execution, reporting, monitoring and evaluation Knowledge and evidence in change management principles with proven ability to foster a culture of change acceptance, adoption and advocacy. Requires exceptional verbal, written and listening skills to comprehend, create and communicate concepts and options to a broad variety of people at different levels of the organization who have different skills and expertise. Ability to develop and facilitate presentations to convey information or transfer knowledge Demonstrated ability to work independently and with a team within a large, complex organization with a variety of staffing groups Demonstrated needs identification, gap analysis, critical thinking skills, decision-making and innovative problem solving Excellent computer skills including expert knowledge of Microsoft Word, Power Point, and Excel required.
Preferred Qualifications:
Masters level degree Formal training and certification in change management, project management and/or improvement methodologies Knowledge and experience with software to support project management, data management and data analysis preferred. Certification in quality improvement approaches such as AHS AIW Green belt, Lean Six Sigma, IHI preferred.
